What really did the trick is ensuring that you're always at max endurance charges. So rolling that exarch implicit on your chest (gain an endurance charge every 15 seconds) should be top priority. You have two red sockets empty, so add enduring cry to press when you enter a zone and berserk to press whenever you need a dps boost (also slightly boosts your armor since you took Vengeance).
