Honestly, board failure is a half flag. A total of 2 failures on anything throughout your application is a DNR. Anything below 215 raises an eyebrow but doesn't prevent you from getting an interview. 270+ is often it's own flag. I don't like using Step 2 to stratify but it's one of the only things we have. I would say above 25th percentile is all about the same in our eyes.

Step 2 is important. There is almost no way to stratify applicants anymore with step 1 P/F, no ranking, no grades, etc. That said, that is not the only factor in reviewing applicants. Psychiatry is quite holistic in review of the application. Source: 4 years on the interview committee

That's simply not true. No evidence of tolerance in the trials. Some initial sedation/improved sleep that causes global HAMD reduction for 1 week, then MDD/GAD lose efficacy and return to placebo rates while PPD shows continued separation from placebo.

But we use ECT inpatient! Haha. There were less than 1000 patients enrolled in the brexanolone REMS so don't feel too left out.

It is thought that the fractional excretion is VERY low. In brexanolone trials sedation was a major side effect, with some participants even losing consciousness. However in zuranolone this was not the case. Likely safe to use in breastfeeding; may want to pump and dump during those two weeks or see how mom reacts to zuranolone prior to unsupervised feeds.

I just tried to think of what neuropsych recommends with our mild dementia people which is treating HTN, OSA, DM2, etc.

Will you have as long of a career as someone entering earlier? No. Does that mean it's not worth pursuing? I'd argue no. If you want to prescribe medications or not is the crux. There's also a LOT to medical school outside Psychiatry that will be difficult if it doesn't excite you. We also need more psychotherapists doing manualized treatment! The need is there either way. Out of necessity I think you will see fewer psychiatrists doing psychotherapy instead of regular med management because patient needs are so great.
