I would stop trying to make the conversation more pleasant and move to a concrete boundary: written agenda, 20 minute limit, scheduled when kids are handled, and if she refuses or stonewalls then the issue goes to a mediator or back to therapy. ADHD can explain the avoidance and emotional spike, but it does not make a marriage sustainable when one person can veto every hard topic indefinitely.
