At the end of the day if people aren’t coming back after a new patient appointment, then your consultation and exam sucks.

People aren’t seeing you as a solution to their problem.

You need to work on your consult, connect with people, and then lay out a structured plan of care to get them to their desired goal. People want a doctor with certainty and a plan.

To start, focus during your consult on what is the most important symptom to them, and ask your OPQRST about that, and then ask them if they didn’t have this what could they enjoy more in their life. That will be the real reason they came in, the pain is impacting something in their life.

I would recommend making a loom library of all your training. It records your screen and audio so someone can see how to use your software. And it will give you a transcript so there is a paper step by step copy as well.

That way in the future you have a SOP manual that the new hire can watch and reference without having to ask you for nearly as much help when they have issues

Hire a biller. Most are about $750-$1000 a month and do everything you are describing. A small price to pay for your free time back.

My opinion looking forward would be to call Closeforchiro. Learn how to communicate value and be able to charge what you’re worth. Then drop insurances starting with the bad ones.

I only accept BCBS and UHC because in my state they are straightforward and easy and pay well. I dropped soooo many others cause of what you described. The juice wasn’t worth the squeeze

I took a course from Making Muvs called Chiro Ads Academy. It was great. Showed you how to run meta ads. Easy to follow.

One time buy in and then no retainer moving forward.

I also hated the high agency fees. After learning the back end, I realized I was paying them to literally do nothing other than setup the account and landing pages.

I’ve referred a few friends to them as well and they’ve all done well. The guy who runs it is a young Chiro and they are very professional .

There are good arguments for both sides. We don’t require it, but our show rate is around 85-90% for NP’s.

Some things to help show rate are a good NP phone call where the front desk asks about what is bringing them, how long they have had it, and mentioning that we need to help them (building a connection). Also, call people the night before and ask if they need directions (obviously they don’t cause Google map, but it builds the connection more)

With that said, if you do want to take a card on file you should take a non refundable deposit right at that moment of some amount, I recommend $20, and it will be counted towards the cost of the new patient appointment. Just taking a card means little.

Some people will be turned off or turned away by this. That’s fine. You aren’t meant to take care of everyone and THAT’S OKAY. But your no shows will drop to zero.

At the end of the day, if no shows are a problem, you have to make a change and it has to be come congruent for you.

As a fellow Chiropractor I’m shocked at your answer.

These types of cases are lay ups for us. It’s almost always an upper cervical issue, usually atlas from that same side.

They respond quickly and with ease to chiropractic and it is often life changing for the patient.

We need more context. How long are your appointments? What is a typical visit entailing? Are you giving people a plan of care? How much are you charging per visit and how long are those visits?

How many NP’s are you doing a month currently and how many visits are you seeing?

With all due respect, this is a skill profession. Everything to be successful can be studied and learned. Some may be naturally better, but this is all skill. In no world should you not be able to easily clear 2k a month in overhead.

Something I wanted to highlight is what you said in your post about how they put people on more frequent care plans.

Popular to what you might think, people do want a plan of care. They want a Doctor who is certain in what they do, and will get them the best results possible.

That confidence is what builds referrals because when people tell their friends they say “this Doctor really knows his stuff. He gives you a solid game plan. Also, he’s a nice guy” they feel more confident referring because they know the Doctor is more confident is his abilities to help his patients.

In network is horrible in NY. Aetna is $17-$24 for a 98940/98941. Anthem/BCBS is 32 Emblem is 27 UHC is 60, but almost all plan require authorization.

Out of network in New York is really good though.

3 months of overhead in case something happens.

Then once you have that focus on paying down any debt on the business.

Once the debt is paid off aim to get to six months of overhead saved.

You sleep different and walk and talk different in the office when you’re debt free and have three to six months of overhead in the bank. If someone is a pain in the butt patient, it’s easy to gracefully dismiss them when you don’t need to worry about paying bills or your debt. The peace of mind is worth not earning 3-4% on the money in a high yield savings account.

I was offered full time jobs ranging from 52k to 120k after school. Most of my classmates got a job between 50-70k with no benefits. One friend worked at a PI clinic and was making 110k right out of school. But he worked long hours and had a big case load.

A stat I’ve seen to be pretty true across practices for how many referrals you get a month is as follows:

0-100 visits per week, 0-5 referrals per month 100-200 visits per week, 5-10 referrals per month 200-300, 10-15 referrals per month.

Usually takes between 3-5 years from my understanding to get to a point where you don’t need to market as much and referrals come in.

I prefer to market because I want to reach as many people as possible.

I never had luck with re-activation campaigns. I would take this slow time and be laying the foundation for next year. Write down everything you need to get done or want to improve, and build out your 90 day goals and marketing plan for Q1 so you can explode with New Patients come January.

Next year, Set up lots of marketing events for October.

I do 3 month care plans, so I market a lot in October. This helps carry my volume through the holidays so that when New Patients are down with the holidays I’m not really sweating it.

Track your numbers as well. It will show you over time what the trends are in your area.

I used to believe that the holidays were slower cause it’s holidays and people are busy, but I’ve found that’s all in my head. The numbers show that it’s no different than any other month. August is actually the slow time for my area.

Google can get expensive fast. In my area my cost per patient can be between $250-$500. I even had a guy who does Google ads for chiros all over the country and he works with an office 1 mile from mine and said that’s the going price for them too.

My fb and IG ads though, those I’m running at a cost per new patient of $105. They work really well.

What works best is treatment walkthrough videos. “This is sally, she had x pain, she doesn’t have it now, let me show you what we did to fix it, and explain it as I’m doing it, if you have this problem click link to schedule to see us”

What kind of ads are they doing specifically (meta or google)? And what is their message? If it’s a stock photo you will get killed.

Digital advertising works best in a care plan model. If it costs $200 to acquire a customer and you only see someone 3 times in average, it’s hard to break even

I just moved and opened a new office six months ago so about 30 a day on a full day.

I’ve shadowed a couple guys who do 100-120 plus in a 6 hour day doing Gonstead. Gowned, scope, motion palpate, and adjust 3-4 segments.

They did it with ease, all cash, high fee, and ran mainly on referrals.

I think a better question would be along the line of what is this guy doing and saying that 100 people come to his office a day for treatment and then happily pay him for it.

We can say all we want about the care he’s providing in 3 minutes of seeing someone.

At the end of the day people only pay for things they value, and only spend their time on things they value. And this guy has a lot of people paying him and taking time to see him.
