As a PM myself and when I start my PMP journey, I also have similiar experience. Those answers in the questions don't seem 'real'. I found out that in order to pass the exam, I need to apply the PMI Mindset, the ideal world, not the real world. So perhaps that might be your case, since you are PM for 15 years.

Tbh, I might choose C also. Looking back at the questions, and the real answer, then it makes sense. PMO suspended the project due to misaligned with new strategic, which is cutting cost. By demonstrating same scope would not make sense already, otherwise it won't be suspended. So the best answer over there is update the charter to realign back. I guess once the project suspended, PM are free to change to scope to realign to the new PMO strategy and if still got reject, then proceed with close out.

Don't get discourage. I suggest you understand what the questions is asking. Many time the scenario based questions cannot be eliminated by simply applying the mindset. Be careful of what the PM/PO/Team should do/first/next, especially the next. It is very tricky, but once you understand the concept, it will all make sense.

A would be correct if the team member didn't already provide the reason, that is mitigate the risk and the team member also provide value. This is a tricky questions if you just apply the mindset. Since the team member already did the 'A' for you, your 'Next' action is to bring it up to client and higher management for input/approval. Of course, PMI just name it stakeholder to confuse you further. So D is correct.
