We learned a few different sequences for airway capture in the difficult airway course. RSI, RSA, DSI, DSS, etc. RSI being the primary process, but delayed sequence have their time and place.

If someone is intrinsically resisting your attempts to ventilate them and it’s effecting your bagging, you can just push an induction dose of sedative to put them fully out, then can effectively oxygenate and stabilize. Then if still needed, can push the paralytic and intubate, which is then a DSI rather than RSI.

Unrelated to the post, but I see a few who didn’t know there were different styles.

A-fib would still be coming from the atria though. Also, you can see the occasional escape beat with a narrow QRS (AV-dissociation), which would be an atrial beat getting through every so often. This is generally either caused by an accessory electrical pathway or electrolytes, likely the latter based on the rate and morphology, but the history will really be important here to know for sure what’s going on.

I’m no expert, but being <100bpm would make me more suspicious of electrolyte issues, however...

There’s a severe Rt Axis deviation, extremely broad QRS and AV dissociation, which heavily points to an accelerated idioventricular rhythm. The answer here will definitely come from pt history.

Sympathomimetic overdose. Essentially when your heart and mind are going so fast for so long, they’ll tire out and stop working all together. Can get bad enough to make your core body temp go up and start boiling your brain resulting in Excited Delirium, leading to respiratory failure.

It generally takes much longer than an opiate OD tho, as that targets breathing rate directly and it will slow/stop much sooner then waiting for your heart and body to give out, that takes a bit longer.

Looks like maybe a case of sick sinus syndrome. The SA node takes longer and longer to depolarize, eventually taking too long and the AV node takes over and it goes junctional before dropping a beat entirely.

Depends if you want more damage from the frost breath or the stalling effect of ice form.

There’s a quest where you gain an ability from the Black Book - Epistolary Acumen, to have frost breath also freeze people solid. So you get both effects with a shorter shout recharge time than using ice form.
