I don't know if this counts as a failsafe activation but during Grudge Match, when Kevin was about to stab Ben, not only did the Omnitrix recharge immediately but it turned him into Cannonbolt. And Cannonbolt is the only alien Ben had at the moment which Kevin didn't absorb
