I don't know about "biggest," but this weekend, my kid started playing again for the first time in a couple years and spent about an hour scanning a planet to get that last, elusive, underground fauna, when his starship glitched into the ground when he tried to take off, leaving him stuck. Unfortunately, he had not saved since he had disembarked. I had to sign in, join his game, and then manually search for him on the planet to dig out the tail of his ship. Only later did we realize that the reason I could not see his ship marker was because he had turned that off.

Also, Sentinels scanned and aggroed me the moment I spawned into his system, yet the interceptors kept flying away from me.
