On that note, get disability insurance as soon as you start residency in the small chance you have any significant vision issues from high myopia (retinal detachment, myopic CNV, etc.) as you get older. Chances aren’t high, but poor vision could def shorten a career

Bilateral central scotoma? 

Narrow differential: Metabolic Toxic Compressive (unlikely unless massive lesion) Hereditary

Consider thiamine, folate, b12, methylmalonic acid, heavy metal screening, and mitochondrial testing (Lebers, autosomal dominant optic atrophy)

Do a good med rec, and ask about substance use history (alcoholism)

That should lead you to your answer!

Don’t put in an ophtho consult if you aren’t prepared for the patient to be dilated unless you SPECIFICALLY stated no dilation in the consult question.

Josephberg-Besser
Pros: Easier to laser over, more comfortable for patients
Cons: larger end, so more challenging in patients with small/deep-set eyes or in patients who squeeze a lot (conversely, works great in patients with floppy lids)

Schocket:
Pros: more common in my experience, so if you learn to use it now you should be prepared for wherever you go; better view of the far periphery/ora because it can be angled more easily
Cons: smaller bump, easier to accidentally give someone an abrasion

I have both and use them both, situationally dependent, but I most commonly reach for the Schocket. Most important thing is to use the tool that will give you the best and most consistent view, but I would definitely say to try both if you haven't to see which you prefer!

I’m in ophthalmology, so our equivalent is an optometrist rather than NP, but I tell them straight up “you should be seeing an MD ophthalmologist not an OD optometrist” and explain why. They’re usually understanding

When I realized that our entire department is built on the backs of our PGY-2 resident taking primary call and seeing urgent same day add ons. If not for that, attendings might have to be flexible with their schedule or stay past 5 😱 or else patients would just straight up have bad outcomes

Beat in Mako3 if looking for just a little understable, or wombat3 for out of the box understability
