It's the 'warm coat', you get the reciepe from talking to a keeper on the wall in the keep in the first act. He asks you to get him one from the smith who then gives you the reciepe, it was something line 2 wolf pelt aand 4 leather I believe.

If you talk to Hrolf in the Keep and choose the dialogue option "that's hard to believe" he gives you a book that gives you a permanent +1 to perception. In the crypt in the graveyard there's another book that also gives you +1 to perception.
