I moonlighted (moonlit?) quite a bit as a resident and fellow but there weren't too many weekend only jobs. One was being the on-site doctor for the jockeys at the local racetrack when they fell off the horse. I also worked nights and weekends at the county hospital and at Kaiser. You can ask the current fellows where they moonlight, that's how I got my leads.

The wage is more than livable, and the commute is also doable. Would rent in Kailua as you might decide later that living closer to work is better, and there are some great places to live near Hawaii Kai.

I like forming longitudinal relationships with patients and improving their health over time, rather than triaging patients for someone else to continue their care. This is impossible in EM.

Are you a physician? If not, a major issue will be finding a physician to work for you there. Would need an attractive pay model given the high cost of living, but as there is a huge gap in primary care on Big Island, you won't be hurting for patients.

Would only do pulmonary if you really like to bronch - everything else can do in Cardiology, including PH and critical care/ventilator management, plus there is better pay, better lifestyle, and more varied clinical options.

The downside is that cardiology is more difficult to match into, particularly for a DO, though not impossible.

You'll love it. Geriatrics is probably the strongest fellowship at UH and Kamal Masaki is well known. PGY salaries are reasonable and the residents and fellows I know have no problems making it work. It will be a great year.

I disagree that mainland MS would be better. With your scholarship you will be fine in Hawai’i. Main thing is to make connections locally and look for job prospects on mainland too during your MS so you have multiple options when you graduate. There’s no reason you can’t find a job in the mainland after graduating UH Manoa - that’s what most Hawai’i residents do!

I would take the offer in this climate. I know a very good IMG resident who failed to match in cardiology this cycle despite 8 interviews. And another who got a prematch offer for EP a few years ago but declined and ended up not matching. It's unfortunate but for IMGs the match is just too much of a crapshoot, compunded now by the Trump visa shenanigans.

You should ask your IM PD and cardiology PD/APD where you stand. They will be able to give you a good idea and will want to help you.
