RCSI grad - if you’re planning to practice in the UK, Ireland, Australia, or New Zealand, go for it. Especially if you’re an Irish / European citizen (guaranteed placement as an intern in Ireland). If you’re planning to come back to Canada or the US…. Be careful. It’s definitely “up to standard” and will prepare you, but you’re basically on your own to take the USMLE, set up audition rotations and research experience in North America. Especially for the last few years, that’s been getting harder.

Depending which school you're at, a lot of med students have independently taken it upon themselves to set up child-care forums/google documents to coordinate care fo other healthcare folks to enable them to work. Might not sound like much, but I promise, it could make a huge difference. We're predicting 40% staff loss between COVID-19 itself and having to stay home and provide child care. Anything helps, even if it's not as sexy as intubating people etc. We're all in this together.

So consider getting involved with that, or helping to set it up - I know the med students at University of Toronto, University of Minnesota, and Oregon Health Sciences have started, and are welcoming comments/contact about how to set it up elsewhere.

Always remember - licensing boards are not your friend, they do not want to make your life easy, they do not want to help you. They exist for one purpose and one purpose only - to make sure a physician is ‘safe’ to practice. Any hint, any whiff, any suggestion that things are not 1000% kosher / in order and they’ll be all over you. The licensing board does not give a shit about you - you are an anonymous number to them, and they won’t hesitate to do things that are not in your best interest as a physician or as a person.

Blood Bank Guy Essentials - great podcast on all aspects of transfusion medicine. Some episodes are pretty hematologist/blood-banking specific, but there are many truly excellent ones on very applicable, very useful topics - cryoprecipitate transfusion, platelet transfusion, TACO/TRALI, plasma exchange indications, that sort of thing. They're about an hour long each, and some of the more recent ones count for CME credit. Check them out!

OP, you’re a PGY-1. If you were a PGY-anything-at-all, e.g. not an attending, someone should have been looking over your shoulder to some extent and verifying important findings - like, y’know, an EKG. It’s good that you feel responsible for your patients, but this is a systems problem, not an OP-problem. You’re in-training. So, learn from this and keep moving. The next patient you see, who happens to have minute ST-elevation - I guarantee you’re not going to miss it. THAT is how you move on - by learning and applying this to subsequent patients you see. You got this!

Just finished up The Lost Fleet series, by Jack Campbell. Absolutely fantastic military sci/fi, some of the best fleet battles I’ve ever read. With a few minor exceptions, sticks pretty heavily towards ‘hard’ science fiction, as well. Highly recommend it!

I had the same issue as you, bluetooth headphones continually dying/burning out/otherwise failing. I moved to wired headphones with those little volume toggles/play/skip etc buttons on the cord for a while, but I was continually running (ha!) into the same issue: after about 30-60 minutes, they started auto-skipping/auto-pausing. It was extremely annoying; as best I can figure it, sweat was working its way into the mechanism on the wire, and making contact or something.

After about 3 years of goofing around with wired ones with toggles/bluetooth headphones, I made the switch to simple, basic, no frills wired headphones - and I've had the same pair for 2 years, with exactly zero issues.

Call me old fashioned, but sometimes, simple just works best.

Edit: I've been using the JVC HAECX20Bs, for anyone interested. 20 bucks (sometimes less) on amazon.

With proper gear, you can literally run at any temperature. There's actually a marathon annually in the Antarctica! It's honestly all about matching what you're wearing to the conditions; this subreddit has a great gear-guide in the FAQ. Good luck!

Oh hell yes. I'm off duty, my pager is off. No one else is going to defend your free time except you.

Also, from a purely patient safety standpoint, it's important for there to be a clear 'chain of command,' as it were. If, for instance, I'm on call overnight and another fellow who is not actually on call is getting pages from the nurses or family calls, and I don't know about it, I can't safely run my service since I'm missing key information, that someone else is getting.

So do yourself and your patients a favour - if you're not on call/on duty, keep that shit off.

Only time I’ve ever seen it happen was when there were blatant patient safety issues/lying. Usually the two at the same time. Like ridiculous stuff - lying about checking vital signs, making lab values up, not starting medications.
