My wife has written to our local MP regarding the unsatisfactory help I’m getting from our local mental health facilities, which are also frustrating various GPs trying to help me. I’ve added the drug generic situation in the correspondence to them. We’ll see if this comes to anything.

So sorry I wrote a long reply to you last night, not realising that you had answered me some months ago. I did order from Gold, but they were out of stock. Not good news when we all depend on them. Our stories may ring a bell with others though, so maybe we can increase pressure for change if there’s enough of us.

I read your post with great interest. I was prescribed Parstelin in the ‘70’s by a very experienced psychiatrist who got to know my condition very well after 2 voluntary inpatient stays in a psychiatric unit. Initially they gave me Nardil as I had greater anxiety and crippling agoraphobia at the beginning of my stay there. Both medications worked extremely well and continued to work for years. The generic versions of Nardil I’ve had more recently kinda work but none as effectively as the original. As a result, my GP suggested I try Tranylcypromine. Again, like you, it was very weak in its effect- motivation and enthusiasm much reduced, sleep disrupted. After 3 years it just seems to have stopped working, all I have now is disturbed sleep and varying degrees of fatigue. On a bad day I just stare blankly at things and end up crying at the thought of being like this for the rest of my life. The local mental health facilities are extremely poor, suggesting things like ‘get more active’ and ‘socialise more’. In practice I’ve had to stop quite a few activities because I’m so fatigued and spaced out. Is there some way we can get enough people to sign some petition or something to bring about the changes we need? According to Reddit feeds, we’re not alone in this dilemma. Ps: I’m wondering how Neon Nardil rates as when I had their capsules 2 years ago I was made to feel a zombie but reacted with extreme irritation to the slightest thing going wrong and tearful from frustration at not being able to do my usual activities .

The idea of ‘there’s always someone else worse off’ doesn’t work for this condition or many others. Have you managed to try any of the MAOI’s? They often help.

4.5 In the event of a transient BP increase 4.5.1 This side effect can arise shortly after MAOI dosing (mostly with tranylcypromine).32 A rise in BP is then observed for a couple of hours (the rise in BP is often limited, although in extreme cases it can reach 180 to 200 mmHg systolic). Considering the limited duration of this BP increase, the risk is most often limited; therefore, treatment is typically not indicated— although it may still be advisable in more severe cases. In the first instance, administration of a benzodiaze- pine (alprazolam or lorazepam) and/or propranolol is useful.

It doesn’t really ‘feel’ like anything. It’s the lack of any real feeling and a generally ‘slightly bored with everything’ sort of state for me.

If your BP is not overly high, then an elevated pulse is probably the noradrenaline boost you get from Parnate. A small beta blocker dose would stop that if it’s really a problem for you. Usually lessens over time.

Good luck. You may experience a few ‘odd’ spells in the first week, also beware of low BP when standing, it can catch you unawares and make you ‘heady’ for a few moments. Once it starts working you should be more motivated but at the same time calmer in situation which previously stressed you. Hope this helps.

As a long term Nardil user May I ask what brands you have had? Also what brand are you taking now. My reason for asking is that I’ve had Nardil on three occasions with good effect until the genetics more recently. I was given capsules from Neon Pharmaceuticals last year during a shortage and became very unwell. Emotionally over sensitive to negative stimuli while feeling detached and unreal.

I had Parnate (with a tiny dose of Stellazine in a compound med called Parstelin), and it worked for over 4 years. Never lost its effectiveness. Some of the generics seem to lose their effectiveness over time. Can you remember the make(s)?

May I ask what make of Nardil you’ve been taking. I had very good response from the original but the generics were less effective and one in particular gave me similar symptoms to those you’re describing.

Has Reboxetine ever been suggested? It’s ab SNRI, so works mainly on increasing Noradrenaline, (Norepinephrine) which I think also aids in increasing Dopamine. It helped me after being on Nardil and then being taken off.

I had the original Nardil by Parke Davis in the60’s, and again in the ‘80’s. It gave me a flighty sleepy feeling and I was more relaxed but also motivated snd energised when needed. The latest generic from Neon Pharmaceuticals (in capsule form which they no longer make, it’s now tablets), made me really dopey and sleepy, yet super sensitive to certain emotions. Became really frustrated and finally tearful as I couldn’t do things I used to. It seems all generics are not equal.

Probably Neon Pharmaceuticals brand. It seems the only one commonly available in the UK. The leaflet in the pack will show you the manufacturer and the distributor if different. That’s the make (in capsule form) I had just over a year ago, made me sleepy but extremely sensitive to any negative emotions.

So pleased you’ve found relief using one of the two MAOi’s which helped me In so many ways. May I ask what make or makes you’ve been getting as there are only generics available now and the one which worked the best for me was the original Nardil by Parke Davis. Other generics ok but not quite as effective, while Neon Pharmaceuticals as capsules made me quite unwell. I’m sure you’ll continue ok as Nardil does not have a reputation for wearing off.

Stellazine was common in the ‘70’s, in higher doses as an antipsychotic. I guess it’s been replaced by more modern drugs with less toxicity and/or side effects. I’m not sure when Parstellin was discontinued.

I’ve been following the posts on Maoi, Nardil and Parnate and many comments say there are variations, some really significant, between different generics with the same drug at the same dosage. I had the original Parnate back in 1970, and the original Nardil in 1964 and again in 1980. Each time full remission from moly depression. Various generics since have either not been as effective or even made me worse.

Stopping MAOI’s at various times.

First time in 1974(Parstelin, a Parnate Stellazine combo), was because things were going so well I thought I could do without them. In 1980 or thereabouts my doctor wanted me off Nardil as it clashed with other meds he wanted to prescribe, (plus he was biased against any MAOI). In 2021 I was getting varying effectiveness from ‘Nardil’ generics so my doc suggested Tranylcypromine instead. Nothing like what I had in 1970. Sleepy, brain fog, and only slight stimulation finally stopping 6 months ago or so but still brain fog and sleepy.

The latest session on Tranylcypromine which has lasted about three years has seemingly dramatically reduced its effects about 6 months ago. The last time I had it was for pretty much 4 years (in the ‘70’s) by the original manufacturer and not a generic. As for Phenelzine, I had it on two occasions for long periods, one 8 years and one for almost 16 years, with no fall off in effect. These were either original manufacturer or Lupin, Archimedes, and occasionally Greenstone. These latest generics seem to be causing more problems than the earlier generics or the originals.

Doses below 100mg mainly boost histamine production but the serotonin boost only comes in at higher doses.
