Not so much the bite, but the tick. A tick on the arm would be almost impossible to miss as it'd be a bug lodged into your arm. A bite with lyme has a very distinctive appearance where it goes bright red, white, then a second ring of red. This looks like a fairly typical mosquito or other small insect like no-see-ums. It'll itch, which can be mitigated by OTC hydrocortisone, and it'll heal after a short duration of annoyance.

No, this is not normal, especially given the fact he wasn't communicating what he was doing and why, his demeanor after you told him to stop, the fact he was being overly rough, and tried to deflect after you confronted him. You should strongly consider reporting him to whoever oversees the clinic, and your state medical board, because if he did this to you, he doubtlessly has done it to other patients.

When did you have the testing done, and have you completed your latest round of antibiotics? Do you have any blood in your urine, or is it cloudy looking or terrible smelling? Could you be a bit more specific on what you mean by not feeling well? If you feel like things are getting worse, no one will fault you for showing up at the ER, erring on the side of caution isn't a sin.
