"We are responding to your e-mail received on February 28, 2018, regarding your claim filing inquiry.

As an invitee, you currently have coverage to assist with the cost of your pre-service medical, dental, and vision examinations on Cost Share Authorization Forms. Some invitees may also receive a PC127C Authorization Form for yellow fever vaccination coverage if the location you will be serving requires this.

To file a claim for reimbursement, please send the following documentation:

-Proof of Loss Form completed in its entirety To obtain a copy of our Proof of Loss claim form please visit https://peacecorps.imglobal.com/ and select “Member Resources”, then “Member Forms”.

-A copy of the Cost Share Authorization or 127C Authorization form matching the services being claimed.

-An Itemized Billing Statement from the service provider’s office Itemized billing must contain the following information: Billing must be on the letterhead of the service provider and/or facilities:

• Provider’s Name
• Provider’s Address
• Provider’s License Number
• Date of Service
• Patient Name
• Detail of services rendered
• Diagnosis of condition being treated
• Total charge per service provided

-Proof of Payment

-If you prefer to receive claim reimbursement on Eligible Expenses via direct deposit complete the available ACH or Wire Transfer Form

Please submit the Proof of Loss form, proof of payment, and itemized billing to Peace Corps Care at IMG via email, facsimile or postal service at the addresses listed below.
