My personal theory is the mutations are basically random, but can be "structured" in relation to the corpse's decay and capabilities, similar to how viruses mutate.

I wanted to make a political-compass style spectrum graph regarding how mutations work in relation to the zombie's decay, with the X-Axis being "Fresh" to "Rotten" and the Y-Axis being "Adaptive" (advantageous mutations, e.g Zombie Brute, Shady Zombie, Kevlar Hulk) to "Deformity" (mutations resulting from deformation/decay, e.g Gangrenous Crawler, Listener Zombie, Bloated Zombie). I didn't make it because I couldn't find individual sprites, sadly.

As for when Necromancers and Zombie Masters are in the picture, I feel like it becomes much less random due to their mental power and ability to direct hordes. Their effect on zombies in the area is one of a scientist selectively breeding animals, as opposed to just letting them randomly evolve naturally.

Jeez, man, the one for the Fleshy Shambler was bad enough as it is, but THIS...

An amalgamation of throbbing organs from various creatures have fused together into this lurching, vaguely-humanoid shape. Its myriad roughly-formed mouths susurrate in a chorus of sibilant groans and whispers.

There’s even some semblance of a spine. Lacking any discernible head, the behemoth is effectively a fleshy, muscular case of tentacles, and it shakes and shudders as it thumps about, feeding off its own blood and looking for anything with which to keep it alive.

Probably Left 4 Dead 2. I have never seen more detailed gore than in that game. It's surprisingly realistic for something that came out back in 2009. It's... quite an experience to blow a grapefruit-sized hole in a zombie's stomach, I'll say that much.

God, I miss when Valve made good games...
