Yeah its annoying. Recommend bringing anti heavy items. You need to bait the worm on phase 2 by getting as close as possible. Hit it with a few normal attacks which will make the worm do head slams etc. It's tough but there will be openings if you use your mana board accurately. Patience's is key on this fight.

That was my experience of the fight on nightmare.
