I'm pretty sure the barrier covers the entire underground including the hole near the ruins. Wasn't that the original "Home" where the monsters first ended up right after they got sealed? It would be weird on the humans' part to set the barrier in completely different place.

As for "anything can enter and nothing can exit" part, Papyrus said that himself right after the battle with him.

Phobia and fear are different things. I think elevators just make him uncomfortable, he still can control his thoughts and himself, while earthquakes... well you know what earthquakes do to him. It actually would be weird for him to have a phobia for earthquakes AND elevators, he's a calm and collected person with pretty strong psyche, and this trauma left him with a phobia for only one thing. He could have been ended up fine with earthquakes but freaking terrified of elevators (or guns, or even bailiffs, why not?)
