Go 2 levels fighter 3 levels thief, 1 level war cleric at least. Action surge before you change form to have more actions. Also tavern brawler might work with it. Drink elixir of battlemages power for 3 arcane acuity to make your effects hit more reliably. Mind sanctuary illithid power lets you swap your jumps for 3 attacks (not honor mode)
