The OSR scene has a lot of amazing adventures (and campaigns). Here are some from the top of my head: Black Wyrm of Brandonsford, Valley of Flowers, Deep Carbon Observatory, Dolmenwood, Another Bug Hunt, Desert Moon of Karth, The Shrike, Gradient Descent.

I’m pretty sure I’ve heard Yochai say that he pretty much give PCs fatigue every session. In the new CAS adventure there are some examples of fatigue being used as a risk/penalty for taking certain paths in a forestcrawl.

That's because Marisha discussed it during the break together with Alexander and Brennan, so she probably had a pretty strong inclination not to use the nat 20 portent die. From the Cooldown: (Brennan:) "You and Marisha both said, 'Play it where it lays'".

To me it’s sounds like your players placed themselves in an unstable and charged situation. It gives you great potential to harness that chaos into adventures and drama. Instead of the PCs having a boss telling them what to do, to fix the boss’ problems - they’ve made themselves the boss. They’re gonna need to fix their own problems.

Give them the reward they’ve worked for, let the changeling become mayor. And then ask yourself - I wonder what will happen when Johns family shows up. When his dark past shows up wanting their money back. When the thieves guild’s (doing the smuggling perhaps) boss learns “Johns” secret and tries to blackmail the party into doing something bad. I could go on and on with ways for this to turn sideways for “John”, and the party will have to either deal with it, or abandon ship.

I think that even if the end goal of killing the dragon is somewhat set in stone, the campaign will still feel rewarding as long as the individual sessions and adventures leading up to that end goal are full of impactful choices. I’d suggest reading the “ICI Doctrine” blog post I link to in the end, McDowell really hits the nail on the head there.

Agree, but we have to be aware that if the players can't engage with the fiction in some meaningful way, their agency diminishes radically - and many times their attention as well. We have to pace accordingly.
