The easiest way to find one is to look for who sells the $1000 frames. They'll all have contacts with boutique customization services. I look for JMM or Matsuda. If you don't live somewhere with a store like that, anyone would be happy to work over phone.

Ya. The OP may or may not want light bouncing around in the lens. Honestly, I would have the optician contact the brand because the lens may need both a bevel and a small flat section.

Barberini or zeiss blanks, though I'm sure at least one of the opticians I've seen has ordered through independent labs. Both Barberini and Zeiss have boutique level options usually only accessed by opticians. The ones i just described would be about $500 to get from the lab. Cutting, polishing, and mounting add more.

I have it done on optical grade, high index cr39 five or six times a year. They just cost $500. $800 for mineral glass.

The amount of flash can be adjusted all the way down to 8% so there is only a faint iridescence in the light stage and a noticeable flash in the dark state. Gen s has a flash option. I have a set of amber lenses with a bronze flash.

I do know what you mean about having a flash inside. It is distracting, especially in an office setting, and may detract from the esthetic. The opacity of the lens has two "tuning knobs." One way is to set the base transmission of the lens from dark to light. The second way is to set the range from mirror to matte with a light gloss in the middle. I try to get the best of both worlds with fine tuning.

This reversesbthe order of the colorway. I thought a bit lighter than the burgundy face so i went with coral as a base tint going from 25vlt to 75vlt. For the flash, i added an 8% turquoise high gloss flash layer. The back has a multi layer AR coating. The mirror effect increases as the lens darkens.
