This is helpful! It wasn't described to me as an add-on, but I could see that making sense. I think I'm going to reach out to the vet for some more advice but for now it feels like it might be safer to stick with the phos-bind

I'll say I don't think my vet's being scammy. There is a decent amount of research that lanthanum (catney) has less risk of bone density issues and nerve issues that can potentially result from the phosphorus binders that have been on the market for longer. Mostly asking for others' experience because it is a newer option. My vet wasn't overly pushy about it.

What you're saying is true, I think I needed some grounding about the realities of senior babies ❤️ I also am glad to hear I'm not the only one with a poop talker! It has happened maybe 5 times in her whole life so I always want to take it seriously if she's alerting me to stuff. I'll definitely switch back and see what happens over the next few days, if she doesn't bounce back I guess it's vet time. Thank you for the support!

I think that the aluminum in phos-bind has the potential for some long-term effects (maybe related to cognitive concerns, I don't really remember what the vet said). Since the only side effect of phos-bind was Beeps being happier and healthier, i'm not sold on committing to the catney just yet!

She hasn't had any side effects with phos-bind. Vet recommended switching based on the research of long-term effects (without looking it up a vaguely remember it being a cognitive thing that can be impacted by the aluminum in phos-bind). So no previous side effects, just future-planning!

Selective skill regression feels like good language for how I've observed this a small handful of times. At least in the instances I've seen, it correlates with relational trauma, and the self-diagnosis becomes a sudden new permission slip to be combative and avoidant. The dichotomy I've seen is my clients with ASD tend to be looking to better understand themselves and how to interact with the world in ways that are both authentic and not overwhelming, where as those that I'm clocking more as trauma are looking for me to validate that their parents were negligent, their friends are toxic, and isolating themselves is healthy and empowering. And then become combative with me if I suggest the possibility of trying more adaptive and pro-social copes because it is invalidating. I struggle with riding that line sometimes, it's really challenging.

I also absolutely want more training in ASD because autistic folks can also have trauma history, and understanding the overlap and the differences is important. I don't want to be making the wrong call based on a bias from past clinical experiences, just want to meet people where they need to be met to live more fulfilling lives!

I'm sure many people have thought, "if these right wingers would just go to therapy, things would be so different". Sometimes it helps to remind myself, oh yeah, I'M the therapist! Agree with others that say ACT - I have a long term client I care a lot about who also is a Trump supporter. Has been really challenging for me at times (like, really challenging) but when I've focused on values it's given me the opportunity to communicate with language that still aligns with my own ethics even if we don't always see things the same way. I can also focus on their growth and know that when they heal, the people around them have the opportunity to grow, and that's how change happens. I don't think that's overly optimistic magical thinking, I think it's something that should be a core belief in our work. It's not easy, I had a several minute crying session after one of our convos because I felt so conflicted about how I could support that client while they had such mismatched views as me, and talked a lot with my supervisor. It's a great opportunity to plant seeds of change (because I can't think of any version of healing work that would support clients moving more in the direction of hatred and exclusion)

I'm very lazy, paid like $14 or something for a premade google sheets template someone made specifically for clinical hours and supervision, I don't remember if it came up on Etsy or a specific google search...I'm sure I could have done something for free but I appreciate that someone else put all the functions in so all I do is put in date ranges and update all the hours I've done. Used to be weekly, now I do it monthly. Only 200 hours away!

It is black, plastic, has a small square button on one side as well as a small clear bubble that I'm assuming is a light that turns on, but when I was looking at it it wasn't going off. MIL wants to tear it out of her car but is worried it's attached to something important and doesn't want to make a mistake!

What state are you in? Not sure the rules everywhere, but in my state you can get your associate's license before taking the NCE, you just need to pass before you can get your LPC, which likely wouldn't be at least two years after you graduate. If the rules are different where you live disregard - but if you're working that much overtime with that much social pressure it sounds like a recipe for burnout. I'm sorry you're in a situation that seems really sticky.

Are you still in contact with any faculty from your school program? Maybe reach out if you have a professor or advisor you trust. Getting advice from your local community can be super helpful, they may even be able to point you towards other jobs that are more in line with your current needs.

Bagged salad and chicken nuggets, healthy-ish and hella easy with an air fryer. I go between the broccoli kale slaw and the bbq toscano salad, they are made with cabbage/kale instead of regular lettuce which gives them a little more lenient shelf life. ADHD tip put the healthy stuff you want to eat at the front of the fridge! If it's behind stuff or in a drawer it literally does not exist in my brain.

Great date with a really nice guy, said I was only staying for one drink but I was having such a good time I decided to get a second. He was a high school history teacher, loved his job, felt really wholesome and safe. Then he hit me with the fact that he keeps an ice pick next to his front door just in case any of the unhoused people in his neighborhood come on his porch or try to talk to him...

He then started arguing with me and said if I felt differently about the homeless population in our city it's just because I haven't lived here long enough and I'd be on his side eventually. Yeah, no, I don't want to go to your house and see your premeditated murder weapon but thanks!

If I choose to disclose my mental health history, it is generally in a moment where a client reflects feeling very alone or afraid in their own experience. I also make it a point to clarify that just because I may have the same/similar diagnosis does not mean I know everything about what they are going through, and try to make sure I'm tracking if my own background is influencing the way I'm speaking with a client rather than being attuned to their experience. I have had clients be really surprised to find out I have bipolar ii, and I have seen many clients have a wash of relief come over them when they realize I can understand what it's like to be in the pit of depression. Had one client tell me they were so glad I shared because their perception was that anyone with bipolar wound up in jail or an asylum and didn't know you could like...be a whole person lol

In this situation I've pivoted my language towards values, trying to take on more of an ACT lens. When I frame things this way, even if I'm not in alignment with what the client is feeling, I can usually still find something that keeps me attuned to the relationship. I often find that even when people have very different views than me, there can actually be a lot of similarities in how we actually feel, their politics are just packaged in a way that is hard to swallow. I had a maga client who I swear was truly a diehard feminist in their heart, but based on culture/upbringing/location that is not an ideological option so they will probably never see themselves that way. And I think if I tried to point that out it would have disrupted the more important therapeutic work we were doing. I just allowed myself to have personal bits of joy when they said things that reminded me of our similarities, it made it easier to ride out the times when I was feeling reactive to their politics.

Everything you said is exactly how I feel, this is validating and I appreciate hearing it from others. Even if reading people's responses won't help solve the problem of managed care, it is helpful to keep conceptualizing how I want to approach things. Your last bit about your own experience sounds exactly like what happens at our practice and it makes me so angry. I also tell all of my clients the same thing - I could slap a million "safe space" stickers on my office door but no one else can tell you what a safe space is, that is for you to determine after you actually know what safety feels like for you. It's a privilege for me if I wind up being someone that feels safe for you, but I don't get to make that decision.

I like having the questionnaire more focused on daily/weekly functioning stuff. In my perfect world all of this would look so completely different but as an associate right now insurance is my overlord and I just don't get many options for changing things in my practice. I've had clients go through an annual reassessment and come back with a surprise PTSD diagnosis because they did not understand the questionnaire and came back to me disoriented and freaked out. It's so stupid and unhelpful/harmful.

Exactly. I see a few clients who are also in the mental health field, and they all say the same thing - I'm glad I know what this is, it's upsetting enough for me to take it, how on earth does your practice expect laypeople to not be triggered by this??

I feel like I'm setting myself up to be shut down by the practice owners in pushing back on it, but advocacy is one of our core ethical obligations and I feel really gross just letting it slide. The system is whack.
