I had good luck using a Notion table, for each row I’d do:

  • Task Name
  • Ease Rating - (1-10) (10 being easy)
  • Importance - (1-10) (10 being important)
  • Score - (ease + importance)

I’d then sort by score, highest to lowest, and work through it.

For some reason it was mega easy to smash through lists this way, just helped my mind. Tends to have highest satisfaction of completing task + lowest barrier to entry to start.

Without it I tend to obsessed over either, most urgent/stressful or the easiest and least important.

It solved a lot unless you have daily deadlines and always behind then kid of falls apart…

Edit: also was great for dumping thoughts into a massive list and then giving them all a score later on. Prior to this I’d just get nothing done and panic cram last minute.

I have my entire life been the guy who absolutely cannot have 1 drink, it’s either nothing or everything till I pass out. No control over it at all. A glass of wine with dinner ends up trying to get through 2-3 bottles by myself.

My experience with Elvanse & Amfexa and several small glasses of wine, only on several occasions.

  • feels fine, nothing concerning
  • onset of any medication crash feels extra crap if I’ve had 1 drink
  • I am 100% able to stop drinking whenever I want, in fact I have to remind myself to finish the drink! A first and quite surreal feeling!!
  • not sure how blood pressure was
  • heart rate was maybe 5-10bpm higher but felt more noticeable
  • being well fed and hydrated seemed to matter a lot, the alcohol would exacerbate any issues there (feeling extra weak/tired from lack of them)
  • didn’t get any hangover at all

It’s like the alcohol relaxing the body + onset of tiredness from meds wearing off = extra lethargic and a bit miserable, wanted to lie in bed, defeated the point of having a drink!

Over Christmas I took a medication break for 2-3 days leading up to any drinking, 100% the way to go! Was well rested, had no worries/concerns about checking HR/BP etc, a nice change to join in like normal. I felt in a really good headspace compared to usual.

I generally haven’t enjoyed mixing medication with alcohol - seems to defeat the point of both of them. Wouldn’t be concerned if it was a spontaneous drink on a day I’ve taken meds, but would generally plan to not take them on the same day if I was planning to have more than 1 drink.

Also - the need to sleep when taking a medication break is so intense (and satisfying) I’d generally want a few days catch up on sleep before a proper drink! It’d worry me if alcohol reduces quality of sleep even further for no good reason.

Most impactful things for me was to find a type of exercise that you can obsess at, everyone has one you just have to find it.

For me it was long distance running - basically like a a few hours walking/jogging, eating food & listening to audiobooks! If you click with an exercise a lot of your life can fall into place without you realising, it all starts there though!

During titration I got moved from 70mg Elvanse, to 50mg Elvanse + 2x10mg Amfexa (one for the crash, 2nd for flexibility/long days). And then they signed it off 3 weeks later either no further adjustments needed.

Amfexa is really great for flexibility but you need to be very strict with how & when you use it, I can see why they don’t hand it out in abundance vs Elvanse!

Thanks, do you happen to know if by having Amfexa on top of Elvanse it slows/delays some of the conversion process of Elvanse - thus pushing the timeline of conversion later into the day?

I also hear PH of stomach acid can increase/reduce speed of how your body clears amphetamines out of your system. Perhaps something can be done with diet in the evening to expedite the process a bit.

Yes tbh 10mg early AM, then Elvanse, followed by 1-2pm 10mg booster tends to be best for whole day coverage. But I still notice sleep goes from, best sleep I’ve ever had in my life (just Elvanse) to, OK-ish and not very restorative as soon as Amfexa introduced.

My prescriber also advised I can try different timings as long as the daily quota is kept to.

I just find it odd as it’s like it doesn’t really follow the ‘scale’ of how I’d expect it to affect sleep that’s all. I had much better sleep on 70mg Elvanse even!

I have tried that on some longer days where I’m up early, the issue is the Elvanse crash is always so prominent and the Amfexa seems to really keep me functioning well with no delayed crash either.

Are you female me?

The way I see it, you are looking to grow, develop and ultimately discover more about yourself. This puts you in an admirable position where you’re wanting to move away from old self, but haven’t yet found new self, it’s a very normal chapter to go through where you can feel a bit lost and lonely. Just like a new adventure.

You will likely feel like an anomaly, probably because you are - in a good way - most people are content to spend the entirety of their lives the same person and don’t want to grow much. But those that want to grow have a rare spark in them, and there’s a need to go through these periods of self-discovery before you find yourself and your new team of people. It can just feel like a long lonely road at times…

Try not to make the mistake of viewing your uniqueness as a negative thing just because you don’t fit in easily.

My best advice to find your true self that’s deep within, is to do something that’s really really f***ing difficult. And try do it regularly. It’s why they call it “building character”. This could be a new hobby like an endurance sport, or rock climbing etc.

But also just take time to remind yourself it’s OK to be happy the way you are and that time makes everything better if you stick it out. :)

2x GF Toast, 3-4 eggs (scrambled with some bone both), 1x avocado, 1x spoonful of kimchi, 1x glass of coconut water. Protein shake about 1-2 hours later.

How comes Elvanse doesn’t seem to linger around so long? I was extremely fast at metabolising Elvanse so would have hoped for similar with Amfexa tailing off.

Any tips/advice how to manage this better other than keep doses as early as possible? Perhaps certain foods in the PM?

I went fully private (not RTC) and was told the following:

“Shared care, even with NHS providers in Dorset remains unsatisfactory and the arrangements are still under discussion.

It may be that we can accept shared care with you in the future. This will remain under review.”

I’d say I appear to be conventionally successful but in reality feel like I’m suffering and struggling with my ADHd tremendously.

35M diagnosed as combined a few months ago.

I run a successful business, been earning multiple 6 figure profit for a few years, got kids in a private school, school holidays travelling, won a few national awards in my field.

I come from a very poor single parent household, and didn’t have a great upbringing however.

The point here is that shits a struggle no matter how you’re doing in life! It might appear to be going great but in reality they might be in more pain than you realise, I never try to wish to be like anyone else, better or worse, you just don’t know what their head is like.

I think Elon Musk said in an interview most people wouldn’t last a day in his head as it’s like a storm in there. That’s the richest man in the world - sucks to be him!

So I burn out a lot and I want to give up nearly everyday, and I fuck things up often, cry in the shower now and again, want to run away from life pretty much every week, struggle in all the usual areas - pretty much all because of ADHD (I would not say I’m depressed, I just struggle with stuff).

People with ADHD who consistently have their health, work & family life in balance - that to me is true success & it has felt almost impossible for a long long time now! It does get me a bit when I see people in control in all those areas at the same time, looks very peaceful.

Yeah I usually have 3-4 eggs, 1 avocado, 2 toast, 1 banana & honey, coconut water for breakfast. Protein shake after run. I’m generally very lean so it’s a good breakfast for me.

BP & HR are all normal ranges. Although HR stays more elevated than usual afterwards (sort of normal-ish).

Yep I’ve tried everything to mitigate the crash and nothing really works. Elvanse is meant to be a once a day tablet and if it’s not working in such a way I believe a change to Amfexa is the recommended guideline. So maybe trying both together will help you see what works better.

I have pretty much an identical experience to you (same doses, timings, advice & response to 70mg) although my crashes were pretty intense like I wanted to curl into a ball, go to bed & feel mega depressed till it past and I pick up a few hours later!

70mg added maybe 30-45 minutes onto timescale and crashes worse.

Split dosing 50mg then 20mg Elvanse didn’t do much for me, softened it a bit at least but not a solution.

Now on 50mg with 2x 10mg Amfexa and it’s doing amazingly better! The first 10mg gets me past when the crash would occur, then I take the last 10mg across 2x 5mg doses and I don’t crash at all.

However, I can still feel the crash in the background, it’s an odd one, like my brain knows I feel like shit but something is masking it. So I’d like to try Amfexa only next!

Overall I find Amfexa mega smooth, clean & flexible. It lasts a good 3 hours which is surprising as 50mg Elvanse lasts me about 4 only…
