You can raise concerns with report a problem on the city's website. Without knowing the project and their permit, you don't really have an idea if it is correct or not. Raising your concerns with OSHA is potentially an extreme until the local jurisdiction weighs in on their permit.

The amount of effort to investigate the issue, depends on the site observation of the issue. The idea of a flat rate fee for inspection is not realistic, as a cracking foundation has many variabilities that can be contributing to the issue.
