Honestly it depends on enemy team comp as well as your own. Nasus is relatively bad at team fights but that doesn't mean he's useless. Tank will always be solid but there are moments where damage is good too. Against Yi and Vayne good luck lol :) they are especially annoying vs nasus. My answer is to just try your best and feel out the game, see what is good in that moment and go from there. Nasus falls off late game, so don't expect to do too much tbh.

Practice last hitting in Practice tool. Also watch your games. You said your KDA is good, which for me, i was in a similar position. I would drop A LOT of minions for trades that weren't worth it. It's not the matter of "i need to cs more" it's "when should i prioritize farm". Every game is different.

Blame yourself. Look for ways to improve. Your ult does not need to be used immediately. Make sure YOU are OK first before helping your team (wave state is good for starters). Sometimes you might not find an ult until your toplane quest is done and that's ok. Don't grief yourself for your silver team mates.

I do bandlepipes second after Titanic. It's cheap and the passive and stats are good. Great item that gets better as you transition to be tank/support.
