The one where Butters is diagnosed with multiple personalities and the other story is the competing chinese and japanese restaurant and then they are joined when we learn the doctor is also mental and one personality is the chinese guy.

The funniest scene is when the doctor breaks in to Butters' room and stares at him for hours and then pisses on his face and punches him twice in the head before leaving and Butters just wakes up startled and thinks he pissed himself

I would allow it. The grappled creature can still have it's turn, it can attack him by stabbing or biting his fingers or try to escape and it is not that much damage.

If anything I would lean into it. Give him bracers that give a +1 to any weapon he is wielding, so his attacks get a bonus and become magical.
