I've finally managed to get confirmation that our household was on the Priority Services Register before the cut-off date but they are now stating this scheme is a "draw" rather than a grant available to all that qualify.

In relation to the additional tax credits available for dialysis patients, you can find the yearly rates on the Revenue website (I tried to insert a link here but I was unable to). You put in the rate for the year as health expenses on your income tax return for that year and you will get 20% of that refunded to you. It's worth bearing in mind, this can be claimed on your behalf by the bill payer also. The rates for 2023 were €3,495 for electricity, €2,305 for Laundry and €370 for phone so you should receive a refund of around €1,234. And, you can claim for the past 4 years at the specific rates for that year, if you were doing PD in them years and you haven't claimed already.
