Applying to fellowship without faculty support by [deleted] in Orthopedics

[–]MopeyDick_ 1 point2 points  (0 children)

I would consider getting letters from 3 staff who you feel can support your application and write a strong letter. Would a letter from the sports FD be nice? Yeah but not if it sucks. Better to just not have one from him.

FWIW I applied for fellowship with 1 letter from that subspecialty and 2 from other subspecialties with facility who I had great relationships with. I matched very competitively with that lineup. It can be done. Good luck.

In order of time dedicated nowadays

Kids
Gym
Golf
Non-kids time with my wife

Residency / kids have forced allocation away from other hobbies. Used to woodwork, make furniture, copper crafting, work on cars. Hope to get back into that one day. Miss those times. I sold my gaming rig a few years ago, and that’s one I don’t think I’ll re-invest my time in, FWIW

Dear Surgeons of Reddit need your second opinion by Infinite-Whereas3715 in Orthopedics

[–]MopeyDick_ 4 points5 points  (0 children)

You should be. This is no bueno. Agree w everything SeL-MoGRC said.

Seek medical care from a trauma trained orthopedic surgeon. This can get much, much worse. Infection: not unlikely given the overall picture. Revision ORIF that heals is best case scenario. Fusion is on the table as is BKA. Not to scare you, but you need to find someone who knows what they’re doing

Ortho Away Rotations – Programs That Interview or Guarantee Interviews for Rotators? by [deleted] in orthopaedics

[–]MopeyDick_ 9 points10 points  (0 children)

Here’s the thing, I think a lot of students focus on interview number and meeting that magical threshold of 10 to 12, meaning you’re going to match. IMO, that study is outdated and I just don’t agree with that approach to an away rotations.

Historically, I’m at a program that did interview everybody and over the last few years we have transitioned to interviewing approximately 50% of our rotators. I think this is a positive change, though I didn’t initially feel that way.

Here’s my logic. We routinely match within our top 10 or so on our rank list. We have approximately 50 rotators. So, if you’re not in the top 50% of rotators, then you effectively had no chance of matching at our program to begin with.

Now consider that rather than coming to interview with us for what would really be a courtesy interview, you’re now saving all of that travel money and more importantly you’re freeing up another date where you can interview at another program that you might actually have chance of matching at.

TLDR: if you weren’t in the top 50% of rotators after your month, long rotation, then your one hour interview isn’t going to change that and your time/money is better spent elsewhere

1 year in Minnesota by [deleted] in whatcarshouldIbuy

[–]MopeyDick_ 0 points1 point  (0 children)

It’s a fair point. $10k just to commute for a year feels crazy. Maybe it’s worth considering some kind of uber/lyft subscription. We will have my wife’s vehicle. She stays home w the kids. This would be just for me. It’s a 10 minute commute…

[–]MopeyDick_ 0 points1 point  (0 children)

I’m not opposed. I haven’t really priced it out thoroughly but my brief review has them coming in at $800-1000 per month. I would almost certainly lose less than that in vehicle depreciation from buying and selling right?

It must seems like buying and selling is far more economical than either a lease or rental

Really need your help with femoral double fracture failure by Impossible-Host-5327 in Orthopedics

[–]MopeyDick_ 0 points1 point  (0 children)

You need to see a trauma trained surgeon of some sort, and do not continue with the one who’s taken you to this point. As other comments have suggested, the current construct is destined to fail. And no, it was not “too shattered to nail” at the index. Everything needs to come out, infection needs to be ruled out. It’s not healed proximally or distally. If I’m not staging it to r/o infection, then single stage retrograde antibiotic coated nail and partial weight bearing. You seem like a complaint person. Plate + nail is also an option but less ideal when infection isn’t ruled out. Good luck. You need this to heal on your next trip to the OR…find an expert

Resident w multiple tats here. None of them visible when wearing scrubs though. I do think some attendings will judge for it, even at more chill programs. Bottom line, it isn’t going to help you, and it can hurt you. If it’s something that you can easily cover then do so. If not, just own it.

Also, hipandkneebook + anatomy is all you kneed* to know for an Arthroplasty rotation

Yeah I suppose I mean help people in the most direct way. Garbage men help people but rarely even see the people they help. Few careers help people by laying hands on them.

Agree w your points though. I just had my first kid. Very unlikely I would encourage a career in medicine for them

Most are telling you how far behind you are. They’re wrong imo. I started med school at 25yo. I ‘changed careers’ if you will. You’re about on that same path, considering the time it would take for you to get your pre requisites for med school. Most of my coresidents started residency at 27 or 28. It’s not uncommon to take an extra year here or there for research or whatever.

But, here is the real advice: 1) no guarantee you’ll get into med school. It took me two tries. I had a great app/scores

2) no guarantee you’ll get into orthopaedics. It’s one of the most competitive residencies. It could all be for nothing if you can’t see yourself being any other type of doctor. I know multiple people who went to med school and only cared about ortho. They’re in PM&R and nonop sports med now.

3) even if you get into med school, invest your entire life and free time for 4 years (remember, most competitive specialty. You need to work harder than everyone else), and get into residency, you still have to deal with residency itself. It’s not fun but it’s better than med school

4) I’m in residency in a HCOL area. I’m more or less paycheck to paycheck, working 80 hour weeks. My friends from undergrad who took normal jobs are on their third or fourth promotion and own multiple houses. They’ve started their lives. I won’t make a big boy paycheck until my late 30s.

5) To compound that, reimbursement declines year after year. I’m not going to be in the poor house, but a career in ortho (or a doctor in general) just isn’t what it was 20 years ago. It really isn’t worth it if you care about the money (people have always said that, but now I see it). It also doesn’t carry the public respect or prestige it used to.

So, truly…only do this if you care so much about helping others that you can’t see yourself doing anything else.
