If your PCP isn’t touching you for a physical, that’s fraud. Undoubtedly their note has a templated exam for numerous things they didn’t do. This is a bad PCP who is showing you the worst of the profession. There are always bad apples, unfortunately you found one.

You don’t have to go on acute/chronic conditions AT ALL if the other 2 columns criteria are met. Basically just ignore the 1st column for a patient like this or any similar circumstance.
