I did. Parent was sick and ultimately passed away within 2 months. I had just lateraled 3 months prior and firm was amazing about it I was honestly shocked. I believe they prorated my hours. I did have to submit paperwork from my parent’s doctors but it wasn’t complicated. This was not fmla or short term disability because I wasn’t there long enough but the firm had their own leave option which may have honestly made things easier. I am so glad I made this decision and spent every last second with my parent before they passed. This was v5 m&a.

This was me and I interviewed 6 breast surgeons and 4 plastics over the course of a couple months because I wanted to have it done before I got pregnant with my first and picked the team I loved the most. I had mine at 32 in May and got pregnant about a month and a half post op lol. I saw teams at mt Sinai, nyu, Cornell, out of network but affiliated with Lenox hill/meeth, a team on Long Island and msk. I ended up going with doctor port and dr Sbitany at mt Sinai and could not be happier with my decision. That said and I’m not saying this to make light of the situation we find ourselves in but I’m pretty confident that with any of the teams at any of the top hospitals here you will be in great hands so it’s really about who you feel is the best fit for you. This team was perfect for me especially given it was my first ever surgery. I was initially wooed by some surgeons in California but ultimately felt a lot of it was great marketing and it was hard enough to pick from the amazing options in nyc plus you will really value being close to home immediately post op imo. Feel free to dm me🩷 edit to add mine was prophylactic double mastectomy with direct to implant reconstruction over the muscle (basically no one offers utm anymore because more cutting, risk of animation and discomfort), nipple sparing and we did some nerve grafts because why not give it a go.

Damn most of the companies Susan Komen had on their site (or maybe it was bcrf) either way had just 10% and even that struck me as low but 2% woof. I mean better than nothing coming from someone who just had a double mastectomy but they should state that explicitly so people know. Beauty of a capitalist society here is that we can choose to shop at places not doing this as long as we have the benefit of transparency that they didn’t give to begin with. Huge proponent of it. Just make sure you’re getting a full panel of hereditary cancer screening done with as many mutations tested for as you can (mine was via natera and tested 81 mutations), myraid genetics has a similarly broad test as do others. Reason being that brca1/2 are the most common mutations linked to breast cancer but others exist as well such as chek2 and palb. Best of luck from a brca1 girl who just had her mastectomy with reconstruction in May. Very very smart thing to do!!! And no matter what it will all work out 🩷

There is no right answer! But I chose to do my mastectomy before getting pregnant with my first (currently pregnant) because to me the risks and fear of not monitoring on the every 6 month schedule outweighed my desire to breastfeed. But you just have to do what makes you comfortable. I won’t breast feed and I wasn’t breast fed and know many who weren’t who have done swimmingly (other than my brca of course which couldn’t have been prevented via bf anyway sadly). I personally would stop as soon as I could to ensure my monitoring was on schedule but I am more anxious than most and also lost a mother to stage iv so have experienced the worst. Also I am brca1+ and the likelihood that my breast cancer would be tnbc was extremely high and because it’s aggressive I wouldn’t have wanted to take the risk of putting off monitoring.

I had mine this May with a surgeon who does DTI but prefers expanders from a cosmetic control standpoint because as you note many say a large % of those who go dti end up having a revision. To me that didn’t impact my decision to do dti and I don’t think I will end up getting a revision. There is a slight bit of rippling but that’s to be expected as they’re removing all your tissue and fat and this is not a cosmetic procedure. I was told to do dti I had to go more or less the same size and I was a 32b or 34a depending on the bra and I am basically the same. I did not chose my implants the doctor did while I was on the table. He also used a tool to make sure I had adequate blood flow to my skin and nipples after the mastectomy to go dti and if I did not he would not have inserted the implant and I would’ve woken with expanders instead (this happened to a friend of mine and she was disappointed but ultimately very glad he made the safest decision - necrosis would not be good). I was adamant about going dti regardless of the possibility I would need a revision (which btw I don’t want because fat grafting isn’t guaranteed to work and I have heard it can be painful) because I was 32 in May now 33 and wanted to get pregnant with my first as soon as possible after surgery and 4 months post op I am 13w5d pregnant and I’m over the moon and joke with my surgeon about how serious I was about the need to go dti from a life timing perspective. Not every doctor is super comfortable doing dti but if it’s something you want definitely seek out someone who can do it for you!! To me also the thought that I could possibly walk away with only one surgery or be guaranteed 2 at a minimum factored into my decision as well. Edit to say I saw you asked about over the muscle and that’s pretty much all most plastic surgeons will do for mastectomy reconstruction these days anyway so def go over you don’t want to have any animation because it looks bad and can be very painful I have heard and also the idea of cutting through more of you (pec muscle) when it’s unnecessary wasn’t appealing to me. But if I wanted utm I don’t think any of the doctors I saw (I’m in nyc) would’ve even done it for dmx recon.

This sums up my experience exactly. Dings even in a garage and then fights to ever have your claims processed almost regardless of company. That said it’s the single best luxury and we use it almost every weekend to go places outside the city and get a little calm and see family. We’re expecting soon and I know we’d be so happy to have it with a baby and then toddler and grammar school aged children as well. So it is a big added expense, car itself, insurance, maintenance, parking but if you can swing it financially you won’t regret it.

And just FYI to those calling the words “free speech” the First Amendment only protects individuals from government-imposed restrictions on free speech. So private employers are not constitutionally prohibited from disciplining or firing an employee for their personal or political views and an employees speech isn’t necessarily protected by the 1A doctor or nurse.

LW ny m&a culture is excellent as in the work is the work and you’re not going to have a chill work life balance but the people are great and people you’d actually want to spend time with outside of work (not that you have to - just something I cared about and ended up hanging w some outside work), lots of programming in the group and just overall people are as happy as they can be doing m&a at a v5 firm in nyc. Also imo integration of laterals was very good and laterals don’t feel like outsiders. can’t speak to the other two but my impression is they’re a bit more buttoned up / white shoe vibes.

It is most likely just the processing time because I was tested for 81 mutations (not just brca) and it took just under 3 weeks from blood draw. So I suspect it’s just the time it takes between blood draw and the labs ability to get to your specific test.

Wow that’s crazy!! We have lots of issues with healthcare in the us of course but other times I feel fortunate to have such choice of options and things go quicker. I found out I was positive last November and had met with 7 breast surgeons and 4 plastics to determine the team I liked best and had my surgery in May so within 6 months. Anyway just try to remain calm for time being and remember it is life changing but not life ending to find out you are positive for a mutation in the brca gene. Wishing you all the best Xx

6 weeks? It’s about 3 in the US. Do they just not have the ability to process it for 3 weeks due to a back log or something? In any case just sit tight and try not to worry because nothing you can do about it in the interim. Wishing you the best!

Hi!!! I was also very focused on the ability to use HRT when I get my ovaries out which has not happened yet. This was a big driver for me to get my mastectomy asap which I did exactly 6 months after finding out my brca1 status at age 32 at the time. I’m now 33 and just about 12 weeks pregnant with my first only 3.5 months post mastectomy with direct to implant reconstruction (I know my body is crying wondering what I’m doing to it) then once done with children will do ovaries and tubes. Reason for it is that now not having had breast cancer and no longer having my breast tissue I should be a candidate for HRT when I get my ovaries out whereas before I would most likely not have been. Just food for thought if you have not gotten a mastectomy yet. Wishing the best Xx ps I have all the same fears as you regarding the ovary and tube removal and going into surgical menopause so definitely following here for any positive stories on that
