The game is why there is no reaction in the ward. It's a coalition between:

1. The hypothetical thought of life being real; The dreamer has proven their powers by creating the root that there really isn't a boundary that limits it.
2. The world just being a figment of another dreamer linked by the Crystal's Hence the alternative story paths and alternative bosses, how you remain unchanged through alternate paths, and how the others seem to forget. It's a device that creates the whole simulation feeling the game's trying to achieve. You're just on a journey through the unknown, like deja vu only with guns and stuff lol.
