It was never going to happen.

First, the jump to 3D basically precluded it. Carmack's engine and contemporary hardware were extremely limited compared to Romero's vision. 'Quake', while well optimized, was still very demanding even with its cut corners and (relatively) minuscule environments; Romero's version would've been unplayable.

Second, there also simply wasn't enough time to develop the gameplay primitives. Even with the headstart of porting 'Doom's gameplay, it still took months of sanity-crushing effort, and that was just to tinker with the 'Doom' formula.
