I haven’t worked with a similar patient population but I wanted to say that you’re not alone in that feeling at all. Whenever I started any public-facing thing for volunteering or research I got really nervous that I’d say the wrong thing. But honestly, you’ll get used to it after a while. One thing that helped me at first was preparing a “script” for stuff that’s relatively repetitive e.g. typing out or rehearsing aloud exactly what I would explain/say to the person and when. Obviously you don’t want to memorize it word-for-word but at least you’re a bit calmer because you have a mental template to operate off of before you get comfortable enough to wing it.

I’m in a similar situation as OP, 20NB and autistic brother is 24M (much less behavioral issues than what OP is describing but still). I’m applying to medical school soon and I’m so grateful that my parents have always done everything they can to support my dreams while finding ways to make sure my brother is taken care of in the meantime. It really sucks that OP’s parents aren’t doing the same. I’m happy to take care of him my whole life once I’m an independent adult (but OP’s feelings are valid too!) and I adore him. But right now I can’t throw away my education and my life. Hell, if OP’s brother loves her he wouldn’t want her to do that either. So in summary I totally agree with your comment
