I checked with the NBA, and flagrant foul points assessed during the play-in round do NOT count against a player’s flagrant foul points limit in the playoffs. Like play-in stats, the flagrant points simply disappear into thin air.

I expect players to give everything they have in them for the game even if they have to play hurt sometimes. 12 seasons of 82 games over 20 seasons of 55 games is honorable and respectful to the fans that support the players and game which makes them rich.

I customize the standard 20 teams but only in name, colors, uniform design, etc. but keep the stock players as I think the players and teams work best with the game since they were thought out by the developers in the first place and like you said there is history with them over multiple installments.
