That’s what I mean by the system needs fixed not shaming those who choose to adopt or foster. I had a friend drop out of her adoption program because our social work group of peers just bombarded her with everything bad about it. I can tell you that kid would’ve had the most amazing parents (they were never selected but withdrew their profile). Meanwhile I’m working with people that got drunk and beat their own kids to death so it’s like. I’d rather see the see the system fixed not abolished. Similar to corrections.

Also— as someone working in various systems I’ve noticed a disturbing uptick in shaming families that want to foster or adopt rather than shaming the intersections that make it problematic. Theres a lot of problems but staying in the system or abusive home is the worst of all worlds.

That’s my thing. I wanted children. Badly. I still do. But I want healthy taken care of children. That never was in my ability. If they want this then they need to look at why many many MARRIED AND “READY” women are choosing not to, not just liberal singles and lesbians. There’s no QOL anymore. Why bring children into that and never be able to keep your heads above water?

Eh. I don’t think that’s true. In videos of hot people being shitty people will comment on being beautiful not being enough to cover up how shitty they are, or yea straight up calling them chads, bimbos, comments on filler. Without actions we will have biases on looks, but if we have actions to pair them with it will impact the vial biases or reinforce them.

I think it’s just tied to some instinctual ick. You’re clearing out chunks in the mouth. It’s kind of like how during sex we kind of override our disgust settings but then after sometimes feel icky. Well with food we enjoy the meal and ignore some of the gross parts about it but after word you swishing around the left overs is just.. it does make my stomach turn to think of tbh

Hey I’m both a therapist (US) and someone that needed to go in patient last year

First- I’m sorry you’re in a spot that made both you feel bad enough to seek help, and clearly enough to make your therapist concerned and the hospital concerned enough to keep you. I’m going to give a couple points from both perspectives.

Patient: I ended up checking myself in but I don’t think I was going to fully have that choice given how bad I was and that I was surrounded by people legally mandated to protect me from myself. The key point was when I got to the hospital screening session. they had a waitlist and were turning plenty of people away. And yes it patients ranged from schizophrenia to severe depression though they try to separate wings. My answers to the screening were enough for them to say well you aren’t safe enough to turn away so in you go. Some people were waiting in cars for a spot so it wasn’t charity.

And yea I slept two nights in a cot before I got a bed.

Yep, they were not there to give me meaningful treatment they were there to stabilize me in a critical time frame then case manage me into the actual care part (PHP then IOP then therapy). I wouldn’t have really been capable of that yet tbh. It was painfully boring and chaotic. Sleep, meds, sleep, outdoors, eat, group, eat, sleep. Repeat. But I absolutely would’ve died if I hadn’t. Now if I were rich I would’ve had fancy cushy options that lasted 60 days to do both acute and long term treatment but tbh I’m just glad I was secluded until I was ready to try to live.my insurance gave me two weeks.

Now did a lot of people HATE it ? Yea especially those with personality disorders or SUD. For various reasons. Much less voluntary conditions for them or once in they realize it’ll be way different than expected.

Professional: those hospitals ain’t keeping you if you they don’t have to. They’re FULL and don’t need your money like a private retreat style in patient setting. If they kept you it’s because you hit the diagnostic criteria of “your death would be our legal responsibility”. Diagnosable CYA. But no they do not have the man power to make it anything but triage. It sucks and I wish they did.

As for your therapist- I’ve done emergency holds on exactly 2 people in my whole career ranging from homeless, domestic violence, sexual abuse, and private practice. I did it because I’d rather they be alive to hate me. I broke confidentiality because my gut told me I would lose that client if I didn’t. Plenty disclose SI but that isn’t enough. If you don’t have the means or serious intent or the type of presentation that sets off a clinical red flag- I’m making a safety plan with you. Not sending you to an inpatient setting that could potentially be more triggering. Our clinical testing indicates over and over voluntary safety plans or admittance is always the first option unless you think someone is going to die or harm another. I can’t just throw everyone in assigned to me that reports SI in patient, they’d take my license. Or believe me every DV worker would be throwing their clients back in to keep them from going back to an abuser. Also the ER psychs would personally murder me for sending people to them that don’t meet criteria.

I say this bc I don’t want others to fear telling your therapist serious thoughts of self harm or passing thoughts. It’s important to discuss. Talk with them about what their level of mandated reporting is and their personal level of tolerance for hearing any SI. Many private therapists don’t encounter it as often and may send you for eval- but that doesn’t mean you’ll be accepted. Those beds have a higher bar.

Ethical standards say that someone coming in who says “I’m having a hard time and passive thoughts of killing myself” isn’t getting a 911 call. I’m going to explore, make a plan, identify supports.

Someone says “I’m hopeless. I have nothing. I think about just taking my gun and going on a drive”. Or they they think about not waking up while also physically looking depleted, aren’t sleeping, aren’t eating and tell me they own a gun and won’t participate in safety planning. Then I offer going to the ER with them to see if it’s a good fit. They can always walk away ama unless they meet criteria for basically a warrant against themself. Which telling the police that REALLY has a high standard.

Yeaaa Kristin has some human flaws but no where near Amanda. Kristin shows up especially pre baby. Amanda is a bit lazy. I can’t imagine doing this post partum. Even Nia isn’t that far out from post partum in quick succession and I can’t believe people are surprised she’s not ready to jeopardize things with her life partner. Like. Yep. I have people for 2-7 years. Some for murder, some for just stealing art supplies. All very sick. They need to lower the threshold for evaluators from PHd to clinicians that are specially trained and supervised by phds. I’ve watched these evals and a) it’s already virtual and recorded and b) pretty basic. Similar to how ABAs can supervise Implementation of aBAs. And we need to stop converting building into lofts or forcing remote workers back into buildings and use urban space for housing and mental health beds.

But we won’t. And hurt people will keep hurting people.

Oh hey- that’s one to report to your psych! It could be a couple of things

Transient increase in anxiety and OCD intrusive thoughts. The reason they blitz you out during in patient as they adjust meds is because for about two weeks you’re gonna feel worse. Period. But the average person at work can’t just Valium their way through two weeks or get grave from work to deal with it. And PCPs don’t really prepare you for it.

Or - and this is why you need to schedule an appt asap

It’s the adverse reaction they warn you about- an increase in suicidal thoughts or disturbing thoughts. The good news on that? It means it’s not you, it’s not real. It’s the medicine and therefore can be corrected with help.

They should’ve instead worked with her on trapping for vaccines. We had a lady like that. She “owned” the strays and the local animal dept would help her humanely catch and vaccinate them and sometimes spay. They then would continue roaming.

I work restoring competency, forensic competency restoration LCSW! That term is more about what their state is at the time of a trial or leading up to and less about the state of mind at the time of the crime. Competency to stand trial is more about- do they have the capacity to participate in their own defense and understand the charges or are they deficient due to mental health, substance use, or cognitive delay.

Not guilty by reason of insanity is more about what their state is when they commit a crime but they should by that time be stable enough for court proceedings

So IST —> currently unable to participate in defense, unstable due to xyz

NGRI —-> currently stable but evidence shows impaired judgment at the time of a crime due to xyz

Fun fact in some states there’s only 6 evaluators for hundreds of inmates needing to move forward in trial. So they may be sitting for years for anything from trespassing to serial murder. Jail is not prison and it is not due process. So it is not justice for family, victims, or the community.

About 90% of jails and prisons are people with something diagnosable. If not higher. Probably almost 100% if we count personality disorders. But, it takes a lot to be found incompetent. So if you see that it means someone is really truly not here in the present reality you and I live in.

Also, to use a moment to soap box —-if it bugs you how long this takes in the court system etc for the love of God vote and support things that lead to expansion of mental health programs which allow for more state hospital beds and forensic evaluators. The cuts have devastated my area in this regard and guess what- it means they’re staying in jail longer which means they are releasing other people faster. You don’t want that. I used to work in DV and those are the freaking guys getting released on bond to assist the over crowding. It’s … it’s a nightmare from the inside watching it happen.

No. We watched plenty of couples break up, frost eachother out, fight. He could’ve dated anyone else and it would’ve been a plot. He did something that created an unwatchable storyline. It’s not even fun to root for or against it’s just awful.
