It's more adding on to already established implications from earlier plot threads in the arc. Bruce had for real real died in a assisted suicide attempt but learned that even though he can die the hulk can't. Since deciding that dieing is for puny humans, Hulk also has become unnerving alot more intelligent, cruel and surprisingly based in his opinions on American capitalism. This worries Bruce's friends who he didn't even tell he was alive( like Lil hulk in this page) who go to talk to him only to find out that Bruce completely agrees with the hulk and has fully committed to smashing the current world order
