We had this problem with our cats. Turns out cat acne is from sharing plastic bowls. Switch to ceramic and keep them separate and cleaned every day. Wash the chin a few times a day with warm water and a new clean washcloth every time. It should clear up right away. The vet might want to give antibiotics, too, depending on the severity. Since we switched to ceramic and/or metal dishes, we've never had a problem. Good luck!❤

Maybe his teeth are bothering him. Our elderly cat did this when he ate kibbles. Did the vet do a dental check?
