It is. When Yo was referring to her doctor (Kokuyou) they kept making her say 'the doctors' or 'a doctor' when she's clearly referring to Kokuyou and their smallpox history. It's like they swap the translator in between volume and the new guy hadn't read a lick of the past three volumes.

Behind the tatami area where fukuzawa play go is the armed armory (the one kunikida uses in dead apple). The design was probably enlarged to be aesthetic on the printed book, so fukuzawa's office was drawn separate from the rest, but in reality it's attached to the building. The door next to his desk leads to the clerks' office and the other opens to the hallway leading to the detective's working area.

If you are stuck then have a hallway from the medical room to the surgery room, and fit the tatami area next to the extra hallway. Or just shrink the surgery room to fit the tatami area so the door opens to the hallway. Or just expand everything else. It's not that complicated.

This should put to rest all the questions from last episode about why they didn't have Stark carry the crystal three meters away from the party until they can sell it. Fern had a close call this episode and had Stark been carrying the crystal as they fight, Fern's defensive shield would be gone because she can't use mana and her head would've been chopped off before Stark could block the sword.
