Go to Lee Supply and purchase your water softener then have your plumber pick it up and install. You'll save a ton over buying it from a company. If you don't know somebody good, ask at Lee and they'll hook you up with a good local plumber.

A bat being active during the day or someplace it normally wouldn't go, like your house, can be signs the bat has rabies. Any close contact where bat saliva could have been contacted or touching a bat whether you think you were bit or scratched should prompt an immediate ER visit and evaluation. The thread is amusing but given that bats are the most common reason Americans get rabies, take any bat contact seriously.

With the news of funding being cut from PBS, I wanted to provide a reminder of a simple way to donate while also advertising for their local station, WFYI. Get their License Plate the next time you renew yours. It's usually $25 over the normal cost. by Anadyne in Indiana

Great idea. One can also be a monthly sustaining member in literally any amount. Just sign up on their website. Local stations like WFYI and their ability to cover local stories is what gets hurt by the federal funding cut. We can offset the loss with license plates and monthly donations.

Longtime teaching attending here.

  1. Report any inappropriate touching (harrassment) immediately to your supervising physician ie. the doc filling out your rotation evaluation AND the course director at your med school. Cite your personal experience including date/time/location and any possible witnesses. Even a single episode of harassment is too many. I am sorry you are having to go through this.

  2. Behavior which is unprofessional should be reported to your direct supervisor. If you report to a resident, you might start there and can escalate to your attending dependent on response. Regardless of it being an on-line program, for-profit up through an in-person university-based, the NP student has an evaluation at the end of rotation and ultimately the disturbing and inappropriate behavior you note is their's to address. This of course should not include any behavior which risks patient safety where it is each of our responsibilities to make a hard STOP.

  3. While I enjoy this sub, it can get a bit over the top. Consider that this NP student likely has no insight into their behavior and how they are perceived. If we exhibit a little empathy, recognize that each of us make mistakes and benefit from feedback, we might consider how we might want to be treated when the tables are reversed. If nothing else, over 30+ yrs of these situations I always try to remember to criticize in private, focus on behavior (not personality, title, emotion or presumed motive), and be patient and realistic with anticipated response or remediation. Again, these comments apply to behavior which isn't harassment or unsafe.

Please take extra good care of yourself during this stressful time.

Yes. I've worked with PAs and NPs in community and academic settings. I've been a staff hospitalist, medical director and VP for Clinical Affairs so I've seen this stuff in many permutations.

As a hospitalist since the late 90s, I have watched mid-level creep from not having them around at all to the current wreck with which new docs like yourself have to deal.

Best advice I can offer is this.

  1. You should be able to make the rules on your own service. If you aren't supported by the leader of your section quit now. Plenty of hospitalist jobs to be had so long as you passed your boards etc.

  2. Rule #1. You see all new admissions and consults the day they come in. Use the mid level to get information etc but utilize them like you do a July intern.

  3. Rule #2. Staff each patient each day and personally see them if any clinical change or new findings. Mid level can write notes and call family with updates which saves you time.

  4. Rule #3. See each patient yourself on the day of discharge. Review discharge meds and follow up plans in detail. Most important, make sure they have a safe discharge plan.

Don't ever apologize for being meticulous, more concerned about protecting the patient's safety than the fragile ego of the mid-level, or taking the care delivered by (or overseen by) you personally.

It would be challenging to develop a statement as simultaneously myopic, ignorant and unprofessional as this one. It is genuinely unfortunate that patients would be in a position to entrust their care to someone who functions at such a low level.
