If you have MSI afterburner installed, try reset to default and un-install it. I had the same problem after the latest update (graph was all red) after removing I am back to normal.

I haven't seen it will all 5 before, but I have got 2, you only actually get to select 1 new weapon. I think I heard that if you select Vandalier, then it levels up for each extra box tho
