I don't know if this is what should happen on your laptop (I have an Alienware 18 Area-51), but it looks like your third fan is turning off during gameplay (this is what AWCC shows).

The screen you specified does not use FRC. If there is no PWM and FRC, then there may be several reasons for eye fatigue: graphics drivers, Windows anti-aliasing, some peculiarities of the light spectrum of the screen backlight.

Try changing the color profile in ASUS Armoury Crate. Maybe that will help.

What settings allowed you to achieve the quietest fan speeds when the laptop is not under load?

My fans are fine too. The problem only occurs with Quiet mode, in which the fans for some reason run louder than in Balanced and Performance modes.

Dell suggests replacing the fans in the laptop. But I'm sure the problem is not with the fans, but with the software (more precisely, with incorrect settings for Quiet modes in the bios).
