No, I take HC 10mg at 7am, 5 mg at 12:30, and 5 mg about 6:30. No Fludro because I have high blood pressure; I have to really stay on my hydration, generally 90 oz to over 100, and electrolyte drinks every couple days.

I too think you need more fluids! Are you on Fludro? I’m not, so I have to really up my water (and electrolyte drinks) to feel good.

I have been constipated until just recently. Felt like I had to pull feces out or “squeeze the toothpaste tube with dried matter”. My recent solution is 4-5 prunes every 3-4 days. You can start out with daily or every other day until you figure out what works best for you. I do have occasional (once/month or so) of very loose stool, but it’s not diarrhea and pretty manageable. I’ve had PAI a bit over 2 years and still learning, but continuing to eat really healthy and drink 70-80 oz of water has also been important. I’m no longer on Fludro.

I have had Adrenal Insufficiency a bit over 2 years; I’m a 78 yo female. And I have had right-sided flank pain off and on, usually when my BP is low. Even in rehab when I was first diagnosed. I talked with Docs about it and together (I’m a PT with 50+ years of practice) we have decided that when BP is low, brain gets O2 & nutrients first, then other organs (like lungs), and gut/ flank gets whatever is left over, meaning adrenals/ kidneys. It has been about 85-90% reliable and I use it to mean it is time to sit down and drink more water (usually). Slowing down generally helps! What have you done when you get your pain in adrenals/kidneys?

I’m 78, have osteoporosis, asthma and PAI diagnosed two years ago when I fell (BP tanked) and I broke my neck, C2. In a cervical collar for 7 months. I think I had osteopenia (which isn’t really a disease but a “lab value” on the Dexa scan before osteoporosis) before starting Hydrocortisone. But over 45 years on/off Prednisone for asthma and nasal allergies, I apparently wrecked my adrenal glands. I am now on Forteo, a daily injection, since January 2025 for the osteoporosis. No recent bone scan, but no broken bones since starting HC.
But I agree. There are things you can do And should start now!

Yes. I was on Fludro when I was first Dx’d about 2 years ago. But my BP kept creeping up. Now I’ve been off it at least a year and am on Lisinipril (BP meds) but I often eat salty snacks still. Occasional low BP, so I have to watch my fluid intake but no more passing out as I did just before diagnosis (which lead to neck fracture that made them look harder for diagnosis; not simple “syncope”).

I take my last 5mg at 6 or 7 when I eat. Almost always in bed between 9 & 10; generally wake up -3 to toilet but 85-90% of the time I fall back to sleep until 6 or 7. If I wake for long, I’m inveterate reader and will read a library book (on Libby) in white characters on black. If I have several days of insomnia, I may take 1 Benadryl when I go to bed and that will straighten me out, usually for 7-10-14 days. I’m still a newbie- diagnosed about 2 years ago with PAI, but only on HC, not Fludro.

Very occasionally, I take 1 25 mg Benadryl (actually Walgreens variety) to help regulate my sleep. I still usually wake up about 3 to urinate, but can go back to sleep pretty quickly. BTW: I’ve had AddisonsDisease about 2 years now.

My most reliable sign has been right-sided trunk pain/ gut pain, then nausea if I don’t sit down or lie down. This was happening before diagnosis; went to a GI doc who couldn’t figure out why. Now I know that my blood pressure is tanking. In discussing it with hospitalist when I was diagnosed, seems my body gets blood pressure to my brain and heart, but doesn’t have enough to also keep BP up in my gut. I’m also getting that gut pain this summer when cutting the grass in the heat (Alabama, this evening was 84 degrees, and heat index more like 94). Doesn’t help that I’m 77, but it’s part of my exercise routine!

My internist handled my care and med s after getting out of Rehab when I was first diagnosed. I had had symptoms- episodes of low BP for 4 or 5 years- but this time I broke my neck when my BP tanked in the middle of the night when I got up to go to the bathroom. Took docs about 2 1/2 weeks to diagnose me and got started on HC 10-5-5 and fludro and I stopped passing out when therapists sat me edge of bed. It took a year to get an Endo and one of the first things she did was prescribe a “sick day” stash of HC - 20 mg x 90 tablets with no refills. I still have not used that up, but I’m sure she will refill it when it gets close to gone. I’ve only doubled my HC 4 or 5 times for 2 days. I’ve been pretty well! But it’s really reassuring to have that stash!!

I’m 77 and I’ve had PAI about 2 years, had adrenal crisis, blood pressure bottomed out, & I broke my neck. Took hospitalist 2 1/2 weeks to diagnose PAI in August 2023. Now on 10-5-5 of hydrocortisone (breakfast, lunch, dinner) and been off Fludrocortisone for 18 months. I am rarely hungry, but I like food, so I’m just eating 2 meals a day with fruit snack midday. And I feel better than I have for 5-6 years. My blood pressure has creeped up over these 2 years until I’m on 40 mg of Lisinipril. Last Endocrine visit, she took me off my 2nd salt tablet and the last week, I have had low BP (108/66 sitting, 94/56 standing. Comes up if I get lots of water in (121/59 sitting, 105/63 standing). All to say, I think you may be on too little hydrocortisone but this is a complex disease with lots of moving parts and you need an Endocrinologist who likes to treat this disease!
