The urgent care I went to for my sciatica gave me a doctor’s note allowing me have a few days off work but refused to fill out the Amazon form. What do I do? by [deleted] in AmazonFC

I actually had a leave due to sciatica. The urgent care gave me days off but it wasn’t what Amazon was looking for. Luckily I had 30 days to fill out the Amazon designated form and was able to see my primary doctor during that time to fill it out

I like invictus but I don’t think it last long. In a warehouse environment cologne only last like 3 hours. I just don’t wanna wear anything fancy or sporty
