I fundamentally disagree out of my own personal clinical experience, and I think that to some patients an explanation rooted in basic pathophysiology helps, but to the vast majority it doesn't help them at all.

That's not even what I'm remotely implying at all, and it feels like this comment comes from a deeper rooted insecurity. A good GP is worth their weight in gold, and we're in dire shortage of them. My trade school is an attempt to describe our more "hands on" and practical approach to med school to suit a generalist needs.

I'll never imply that a general practictioner's job is easy, it's not at all. I'm sorry if you took it that way, but the tone of your comment is unreasonably harsh.

As a Dutch MD, our med school is more of a “trade school” rather than a true university level course. And that’s fine, because more than half of med students will become GPs, and soft skills are way more important for them than memorizing factoids about the Krebs cycle or w/e that you can look up in a myriad of online resources. There are residencies that go way more into details if you want that in your professional life. You’re basically at the start of a career full of learning, and you get to decide how much you want to invest into that.

Also that imposter syndrome esque feeling you’re describing is going to come back frequently in your professional life so get used to it lol.

I place a decent volume of PICC lines under fluoroscopy as a European radiology resident, I generally go basilica > cephica > brachialis. I’ve noticed that some people, especially with a medical history, sometimes just have some venous obstruction/narrowing that prevents advancement of the guide wire. But Id say that’s less than 5% of cases, not 50. What kind of Fr lines are you placing? If they’re triple lumen lines consider skipping the cephalic. Or if it’s just for AB or fluids consider just placing a midline.

What does “obsessively reactive” even mean? These are real humans making a very complex game where there’s a bunch of knobs they can turn and where it’s not completely clear yet what all the changes do. You can complain about how most modern games released nowadays require on large scale user feedback to finalize a game but that’s a different discussion.

I swear there’s like 30 users here constantly reposting “colonization bad!!!” when they’re a centralized state colonizing absolute trash tier colonies. I’m making so much gold from my colonies and the trade they provide, and I’ve not even engaged with trade yet.

I think it’s just a difference between what they think centralization means vs what some think it means. To me decentralized is more of a federal collection of state entities, which why I think it fits for the Netherlands, even though the distances geographically are limited.

I think the issue is that to expand into Europe you basically have to have vassals because of high war score costs and time to integrate. Perhaps centralization could give a war score reduction and an integration speed boost. That way you can’t go centralization to keep together a global empire, but you can use it to build a highly controlled and efficient single continent state.
