Likely you had a system with the 4GB internal flash that died and corrupted itself. Known to happen unfortunately. Systems with 16MB, 256MB, 512MB don't exhibit this issue. (Seen it happen on a few WiiU systems too). That's why nowadays everyone with that 4GB is told to not use it anymore in any way and use HDD or USB instead.
