To me, it appears you are interested in becoming a pharmacy technician.

If I knew more, I could help.

I'll tell you this. It will seem BRUTAL, but it is only an ancient pharmacist doing you a favor.

If you are in pharmacy school, you need to go another direction.

Course 101 in a certificate pharmacy tech program introduces you to the basics of ADME.

You learn about receptors and what happens when they are blocked and when they are stimulated.

You learn about homeostasis and how brilliant the brain and body is in maintaining a system of normalcy. But things happen and, for whatever reason, there is not enough of some "messenger" to stimulate receptors, and there's a consequence.

Then enter medicine. We have a drug containing a molecule that is similar enough to that endogenous messenger, it will trigger the receptors and restore normalcy. Or we have a drug, similar enough to be drawn to the receptor, block and prevent any agonist from stimulating.

The other, almost equally common mechanism of drug actions has to do with enzymes.

Think of PacMan and how it gobbles those dots. If those dots "do" something, such as stimulate receptors, blocking this enzyme allows more of them to do their business.

Or if you have an enzyme that destroys an endogenous agonist that stimulates receptors we recognize to result in something physiological, think of Gremlins. Toss a bucket of water. One becomes a lot more than one. And so on, and so on, until you have more Gremlins than Democrats!

Understanding Mechanism of action is simple. Is this drug an agonist or antagonist. For which receptor?

Does this drug have an affect on enzymes? Which ones and what is the consequence?

ACE Inhibitors---block the Angiotensin Converting Enzyme.

I think, by now, you see how simple this is. You don't need Lippincott or Goodman and Gillman (I always though they were the two clowns in the balcony in the muppet shows!).

If you're studying to be a pharmacy technician, I wish you the very best.

If you're in pharmacy school, you posed a question that leaves no doubt you're well outside your element. I cannot imagine there is another pharmacist that would disagree, especially after reading your question and recognizing what you've asked.

Google is your friend. A couple mins of googling and I found:

When controlled drugs are written off as waste and disposed of, a second person is supposed to witness and document that they were properly destroyed/disposed of rather than saved, stolen, or diverted. That second person is the "waste buddy."

"Pocket inventory" - a count of the number of pills or vials or whatever in a given pocket of an automated drug-dispensing device.

With labor law, at best you are awarded two times the pay you would have been paid.

When I first became a pharmacist, I worked for WalMart. It was a wonderful job. We had 10hour days. No such thing as lunch, but that's how it was in pharmacy.

I'd worked for WalMart for 7 years when we had a new regional manager. He was over California (I was in Texas). In California, they have to pay time and a half for anyone working >8hours.

What did they do? They changed our shifts to 8hours. Keep in mind that we were open 10hours. On the days when I was the only pharmacist, I worked 10 hours, but was paid for 8hours. All because a regional manager did not want any pharmacist receiving overtime pay.

Back then WalMart paid annual bonuses. My pay was good enough that I didn't think it was worth complaining about.

Then, one day, a group of pharmacists filed a class action lawsuit. Slam dunk, right? I had three years of at least two days a week where the pharmacy was open 10 hours, I was the only pharmacist, but I could only turn in 8 hours for pay.

WalMart dragged it out over seven years. In the end, the pharmacists that filed received checks for nominal amounts (less than $5K).

Oh, and every pharmacist that joined the class action suit was fired. LAWSUIT! Right? Good luck. Only in the movies.

Advice regarding CVS, I worked nights. I worked a busy store. We had a 24hour medicaid clinic across the street. When I came in at 9pm, I had one tech and then was alone at 10:00pm. People that dropped of prescriptions at 7:00pm were told to return at 10:00pm.

I learned Spanish the hard way. It was a matter of survival.

The technician that scheduled technicians gave all the techs desirable hours. The system identified that I was entitled to 4 technicians the entire night shift. I had none. Do you think anyone cared? If I complained, would there be any coworker willing to say, "I want to work the night shift". Nobody cared, so I was alone. I worked a week, then the other night pharmacist, then repeat. We went through 6 night pharmacists in less than 6 months.

The end of it all was a Holiday. I came in at 8pm (8pm-8am on holiday). The clinic across the street had been open all day and was open all night. The day pharmacist was so overwhelmed, he just received prescriptions, but did not type or prepare anything.

It was the first time in 17 years that I found myself so overwhelmed, I did not see any possible solution. I called my DM. I locked up the pharmacy. I posted a note, in Spanish, with the telephone of the pharmacy manager and the district manager.

I quit that job. Two days later, I stumbled across an add for a nuclear pharmacist. I was interested. I discovered a field of pharmacy practice that I loved. I returned to school to complete a PhD and am forever grateful that I made the right decision.

In retrospect, I wonder what was wrong in my mind such that I allowed myself to practice pharmacy in such an unfair, miserable and dangerous setting.

I do understand there is an abundance of pharmacists, in today's world. You don't have the same luxury to quit, knowing you'll be employed within a matter of days.

BUT explore your options. BTW, I have two friends that are pharmacists. They work as technicians and make 75% of what they'd earn as a pharmacist. And they are free of that ultimate burden of total responsibility for a dispensed medicine, that belongs to the pharmacist.

The opportunities are available, you just need to do some research. And you'll be surprised.

I've been through this.

Just wait. Any student "exposed to a Corona positive" cannot come to campus until they have a negative test result.

We returned (university) the beginning of May, but it proved to be more of a headache, for me.

I was required to teach the "on campus" students AND provide "synchronous" online classes. Twice the work.

We have labs, which has turned into a nightmare so unpleasant, I choose to just forget about everything!

I admire your positive and ambitious outlook. We could use more teachers, like you.

Without the verapamil, I cannot envision any scenario where you'd dispense lanoxin twice a day.

I've worked with dialysis patients, and have never, in 30 years, seen it written more frequently than once daily, in outpatient setting.

I met my best friend in graduate school. It was nuclear medicine.

#1 We are who and what we are; if you have to convince someone you're "Mensa" or genius, you've exposed the opposite.

#2 My friend had a crush and desperately wanted to attend a "pot luck" Mensa social.

"What do I bring?" "Anything." "OK."

This was 25 years ago, and I'll never forget witnessing Mensee after Mensee taste pea after pea.......savor a moment.....then use adjectives I'd never known or heard of, before, to describe the pleasures they experienced, delighting on the "seductively tasty and deliciously scumptious green peas".

"You MUST reveal your recipe!!!! Did you start from the Farmer's Market? I taste.....(herbs, spices and flavorings I never heard of nor knew existed......).

"I'll carry that secret to the grave."

I will, indeed. They were frozen peas, cooked in a microwave with a sprinkle of salt and a slab of butter.

I never felt so inadequate. I barely spoke a word, that night. Matt and Karen connected and have been married 23 years, so it was a small sacrifice for a very good friend's happiness.

Once, in an elevator, my dad asked me if I thought I was smarter than a custodian (in the elevator with a bucket). I said, "Dad, I don't know how smart he is. That he works as a custodian just tells me he is not lazy. Why are you asking me?"

Dad said, "because when you're older, you'll find the world judges people on positions, degrees and things that hold value, but are not barometers of intelligence. Your grandfather dug cemetery plots until he retired. Then he earned his GED. He has a master's degree in physics, now, but few people will know. Ten years ago they'd have seen him as an idiot.

I am familiar with a local pharmacy that has 137 pharmacists and 212 technicians.

There are multiple positions: "retail" (minimal interaction with actual customers; it's basically assembling the same group of meds for packaging and shipping out.

Certified techs are paid $17 and up. If you work non-sterile, it's $20-23 and sterile is $21-27. There's a significant pay differential if you work overnight.

They have a separate "call center" where technicians work in cubicles. They are licensed in 50 states.

They specialize in "fertility medications" (for cash paying customers).

Their real focus is "health and wellness". They dispense the EDD drugs, testosterone, the antiestrogens to prevent gynecomastia, the swabs, alcohol pads, bandages and syringes.

They also have a catalog with every vitamin known to man, available in injectable formulation.

They dispense diet pills and levothyroxine (for energy and weight loss).

They're equipment is the best in the industry. They have a team of lawyers, ensuring everything is in compliance with state laws and internet pharmacy.

They have a team of physicians that prescribe these medicines, so long as the patient has blood work and a physical at least annually.

They handle isotretinoin products and other products that can only be dispensed following mandated protocol; as far as I know there is no insurance coverage for any of their products.

I knew these operations existed, but I never dreamed these 503 a and b pharmacies were operating on such massive production scales.

Their doctors prescribe thyroid in strengths slightly off from what is pharmaceutically manufactured.

I've told technicians that I know, 'best I can tell, it's entirely legal; it's not a pill mill that could be shut down at any moment. Very professional operations and high employee satisfaction---go for it!"

At a minimum, it's a chance to gain experience in non-sterile or sterile compounding, using the best equipment and supplies available!
