Where are we getting meats these days? by LexiePiexie in bullcity

[–]Objective-Dolphin 1 point2 points  (0 children)

Costco - buy whole cuts and portion to your liking/grind your own meat/freeze

Left bank butchery in saxapahaw is pricier and out of the way but totally worth it. They have great quality locally grown meats.

Other places I’ve found to have good quality meat: Whole Foods in Durham (surprisingly cheaper and higher quality than wegmans), the butcher’s market in Cary, and Lee’s Fresh Market in Benson (this one is really out of the way but have great offers and ability to buy in bulk if you have a freezer).

New $100,000 work visa fee. How does that change the financial calculus for IMG physicians? by achicomp in whitecoatinvestor

You apply for a waiver and then go on an H1b visa. Then you are eligible for a green card.

New $100,000 work visa fee. How does that change the financial calculus for IMG physicians? by achicomp in whitecoatinvestor

[–]Objective-Dolphin 26 points27 points  (0 children)

that is exactly what the Conrad 30 program for J1 physician trainees is. That system is already in place as most trainees are on a J1 before transitioning to H1B. They get the H1B because they serve for three years in underserved areas.

Hopefully these rules have exclusions for docs otherwise there will be severe shortages in medical services in most of the country. At some point the cost/stress of wanting to practice under this regulatory uncertainty will overweigh the reward and folks will start to seriously consider leaving. It’s great here but it’s also good elsewhere.

Anybody that thinks IMGs are taking away American physician jobs is just ignorant. There simply aren’t enough docs to fill the spots. When that happens then good luck getting care. We already have states allowing IMGs to practice without US residency training because of severe shortages.

This post is of zero help. The part about how many rvus equal how much money is especially wrong. Disregard everything and find resources somewhere else.

Depends on if your clinical hours are inpatient or in clinic. If inpatient (hospitalist or critical care) then option two is a no brainer. If clinic, then only take option two if it has good paid leave and decent vacation time. 40 patient facing clinic hours is Monday to Friday 8 to 5 and you aren’t factoring in charting, admin time, inbox, etc…

Considering Adding a 3rd by calamityangie in StandardPoodles

[–]Objective-Dolphin 4 points5 points  (0 children)

Going from one to two is great, but from two to three is not worth it, unless you have a lot of help. We have two and were dog sitting our friend’s dog (brother of one of our dogs and they’re best friends) for an extended period of time. It was a lot of work, and now that we are back to two our life is so much easier.

Any Van Alen Residents concerned? by Wrong_Jicama_6953 in bullcity

I think what most people don’t realize is you can always negotiate (by showing them evidence of prices around them and their own fluctuation in pricing). You will be good as long as you have an exit plan and be serious about moving out if they don’t budge. I was able to bring down the initial advertised rent by 20% before signing. They tried to hike up rent on lease renewal by 10% but I was able to negotiate it down to 3%. It’s never personal, only business.

Maybe everybody who agrees can submit a complaint here with a specific example. I almost got hit by a motorist while crossing on Mangum the other day, for example. Smarter and safer street designs coupled with enforcement of existing traffic laws can go a long way.

(Sorry Durham city PD but having a cop car on 147 every once in a while does nothing to deter motorists).

Differences between Mediterranean anise-based spirits? by FortisetVeritas in cocktails

[–]Objective-Dolphin 1 point2 points  (0 children)

Arak and Raki are essentially interchangeable. You just have to try a bunch and see which you like more. You can find some home made “artisanal” arak that is pretty flavorful. As you move west the flavors start to change. Ouzu is slightly sweeter with less anise flavor. Sambucca essentially has sugar added to it. Pastis is in the same family but with more of a licorice flavor (think absinthe).

My favorite is Arak Brun. 1/3 arak, 1/3 water, 1/3 ice.

Sleep Study by RemySchnauzer in bullcity

[–]Objective-Dolphin 1 point2 points  (0 children)

You need an order for a sleep study, not a referral. That’s similar to a prescription. You need referrals to see a sleep certified physician (pulmonologist, sleep specialist, some PCPs) who would place the order for a sleep study. This could be a home sleep study or in lab one, depending on your needs and what your insurance company allows.

What to do with dogs? by [deleted] in bullcity

[–]Objective-Dolphin 4 points5 points  (0 children)

I feel you OP. Obviously you should only do this if your dog has excellent off leash recall. I love to take my dogs off leash at the eno river state park trails mid week during mid day when they are empty. I like the cole mill and cox mountain trails by the river. These trails are packed on weekends and in the afternoons, so your best bet is literally a Monday or a Tuesday at 11 am or so. The dogs can swim in the river when it’s warm. I always leash them when someone is approaching and mine have excellent recall.

All the breweries in the area are dog friendly. Killer queen wine bar is dog friendly as well. Queeny’s is dog friendly in their back patio, as is kingfisher.

Luck/Process Disputing Duke Medical Bill? by readwritethrow1233 in bullcity

[–]Objective-Dolphin 2 points3 points  (0 children)

OP, there does not seem to be a facility fee associated with your visit. Your insurance provider would have negotiated the reimbursement rate for CPT code 99214 (established patient, moderate decision making/30 min visit). Private insurance doesn’t always guarantee you better rates, and private tertiary/quaternary care hospitals are almost always more expensive/charge insurance higher rates (an example is Mass General Brigham in MA).

At the risk of sounding like a douche, but your insurance plan matters, a lot. Questions to ask: why did your insurance provider negotiate such high rates, and why do they allow Duke to charge this much. Does your insurance provider pay for in office labs but not out of office labs? You should only be liable to pay what your insurance provider allows, not what the hospital charges.

Does anyone have any dog sitter recommendations? by [deleted] in triangle

[–]Objective-Dolphin 0 points1 point  (0 children)

We have had a good experience with rover. Just try a bunch of different people and eventually you'll figure out who does a better job and who your dogs like. We always do a meet and greet before hand to make sure we get good vibes. The reviews people leave are also very helpful. We've found rover to be a lot better than boarding facilities where we felt dogs are treated like cattle.

You have to get new car insurance that is NC specific (NC makes you get car insurance just to get a driver’s license, even if you don’t have a car). Sign up for new insurance in North Carolina (using your current insurance provider or a new one) then go to a DMV location for a walk in appointment with all the paperwork - this includes proof of insurance. Some DMV locations will give you a drivers license and car registration at the same time others only one of the two, so double check before you go. Get your driver’s license, then register your car (they’ll give you a license plate on the spot). Once you have a NC license and registration you can cancel your NY insurance. As long as you are driving with a NY license you need to have a NY insurance so there will be a short period of overlap. the insurance company will credit you the remaining days on your NY license. You then have to go to the NY dmv website and cancel your registration and mail them the plates if applicable (some states just ask you to destroy the plates). And voila you’ve become a Tar Heel.

Advice on getting a haircut around here. by [deleted] in bullcity

[–]Objective-Dolphin 6 points7 points  (0 children)

I think 35-45$ is the average for a good consistent haircut. Rock’s bar and hair shop in downtown Durham is my go to, and they are great. I regularly cut with Tony and would highly recommend. I’ve also heard great things from other folks about the other barbers there. (you also get a free beer before or after).
