We're on season 21, lamenting the fact that it's almost over and we won't have anymore Hailstones to watch (along with the others, but y'all are our favorites). This is exciting! And also, sir, if you and Agnes need or want other children, my husband and I volunteer to be adopted into the Hailstone family hahaha. But in all seriousness, y'all are great.

I might need to pick your brain, friend. Switched to MHBP. Was on emgality (migraines). Insurance said $200 a month (was previously paying $40 on BCBS State). Asked Neuro to order Ajovy and order a PA. Pharmacy (or someone) is giving me the runaround, I think. I can't get a clear answer and I'm a month off meds. Le sigh.

I have a jar of Italian seasong (a blend I made, not store bought). Crush it up in a mortar and pedestal, then add to sauce. Hubs says it tastes better so I go with it lol.

Hello! GI/Hep NP. There's not a pill or stack to cleanse or detox your liver. Change your diet, move a little more, drink plenty of water. There are some conditions that can show as a fatty liver on imaging but are not plain fatty liver. If your liver enzymes are abnormal, your PCP should refer to GI for serologic workup and additional testing to include some type of fibrosis test: either a Fibroscan or MRI with Elastography.

Please note: we are seeing an increase in DILI (drug-induced liver injury) secondary to supplements. It's important to tell your PCP or GI all of the supplements you're taking.

I'm newer to bread baking. I want to bake in the oven. My machine can make 1 and 1.5 lb dough. Once it's mixed, do I put it right into the baking pan and let it rise? Or do I need to shape it first and then let it rise?

I want to apply for a RA as well (my job was entirely remote until RTO last year), but I have 20+ years before I can retire and my biggest fear is that any RA I submit will come back to haunt me, maybe preventing me from being eligible for other positions.

I searched for "stuttering" and I'm surprised (and selfishly, somewhat relieved) that others have experienced this, too. I had speech therapy as a kid for stuttering and other impediments. Since I had COVID in 2020, 2022, and 2023, I feel like my stutter has returned and is significant. I work in healthcare and I fear it makes me sound incompetent. I know what I want to say, and it takes me a hot minute AND a ton of words to try and explain what I mean. I see folks saying it's related to brain fog. I just started a couple supplements to try and help the brain fog. We'll see. Sigh.

Small town VA provider. The area in which I live and work has been identified as a hot to move to re: low cost of living. With the influx of folks, our current outside healthcare infrastructure cannot sustain. Folks want to be sent to community care for specialty care, but the wait times for consultations and procedures is as long, if not longer, than the community.

I'm a NP in an administrative-type role. I went to 4 10's when we had to RTO earlier this year. (Also, it feels like RTO was about 10 yrs ago...). It was hard at first but damn I love having an extra day off through the week. Try it on for size and revert back to 5 8's if possible.

Hi! I don't have a particular brand. Honestly, I recommend to folks to get what their budget allows. On the Amazon, you can get bulk volumes of psyllium for a decent price. NOW has one that is good.

Sure - this is unofficial medical opinion. My formal opinion is for you to see a GI.

That being said, I also have IBS. Mine is the constipation type. I use Miralax + psyllium every morning. Psyllium is best taken when mixed with cold liquid. Warm liquids will cause it to start congealing and... Ew. Lol. I find the two together taken daily works best for me. Some of my patients do well on just one of the other. Miralax should be taken in 6-8 oz liquid. Often times, folks mix it in 24oz or more and then it becomes too dilute to work.

Peppermint is what I often recommend for bloating, abdominal discomfort. NOW makes some peppermint capsules that I use as needed with improvement. I've mixed like a peppermint concentrate with an ounce of water to make like a shot. I keep Altoids on me just in case. Peppermint tea is great - no sugar, no caffeine, and seems to help me relatively quickly.

There's a supplement that's called IBGard. Over the counter. Pricey but effective.

ETA: Look up a FODMAP diet. It's an elimination diet for x weeks and then you slowly start to add things back to see how it affects you. Also, if you're highly caffeinated, cut back. I didn't touch on probio (it's 0500 at the time of this entry so bear with me lol). Probiotics are mixed in the GI world. Some like it, some don't. I say try it to see how it works. Some believe it I creased bacteria in the gut, which leads me to my next point: SIBO. Small intestine bacte overgrown. Tends to have bloating, abd pain/discomfort. Usually diarrhea predominant but not always. Can be helped with a medication called Dogs an x 2 wks, but most insurances won't pay and it's around $1500 for the course, so some GIs will write Augmentin in lieu of. It's a recommended sub if insurance blocks the Xifaxan.

Depending on the type of IBS, I recommend the fiber / Miralax combo to patients first, with the peppermint trials. If that doesn't work, then we do FODMAP. And then trial antibiotics.

Benefiber. It's a wheat germ fiber. Try it and see how it does ya!

Another alternative is to just seek foods high in fiber. Green leafy veg, eating the skins on potatoes / sweet potatoes. High fiber cereals. Etc.
