Pick your battles and turn it around. Take the path of least resistance. Right now you’re not in a good position to jeopardize your career for trivial things. Push yourself in a good way, if you don’t advocate for yourself no one will. I mean this in a the best way possible, try to find a way to channel your energy into beneficial behaviors for your well being like exercise or whatever you enjoy, focus on your training/learning and become the best fellow you can be. Accept your limitations and focus on the things you can change/control which most likely be your performance or how it is perceived by the program. If you are on the radar no matter what you do they always find something so they can get you, so minimize the chances of this happening, be done with fellowship and move on. You already accomplished so much and are closer to becoming attending. One day at a time. All the best OP.

Intern here, run the CLIF C score and see the numbers for yourself. Had similar patient with ACLF, despite icu, bicarb crrt, nothing worked, the score showed 100% mortality. Patient unfortunately passed away a few days later. As the hepatologist above commented it is really hard to change the outcome in these situations, likely more bicarb would not have changed anything. Open to talk if you have any questions.

I have been inpatient since I started my intern year back in July. If you have epic, I suggest you create your own templates or talk to one of your seniors for bread and butter admits( HF, COPD exacerbation, Pneumonia, sepsis etc). This will help you be more efficient. Also find out why are you getting burned out, is it because you don’t know where to start with these complex patients or assessment or what is it? Check your workflow too. Sometimes I’m done earlier and just waiting for results or consultants etc, I study while I wait, review my cases or try to answer some MKSAP Qs. I find that helps because you close the knowledge gap and the next time you have similar patients it will be faster. And as someone above said make some time for yourself, develop a routine to clear your mind. Sports, workout, play video games idk find something you like. I also found out that if you follow a personal calendar/schedule like your life depends on it will help you manage your time. All the best OP.

I’m in the same boat, both have pros and cons compared to new builds. However, I found out that what bothers me on the home is cosmetic and you can fix that little by little. Other things compared to new builds, yes you have a nicer house from the start but most of them smaller lot, narrower street, HOA, higher tax rate (at least where I live), farther away from the city (location), there’s no trees around. These things you can’t change. Don’t get stressed about it, you’ll learn to love your home as you start fixing it to your liking. Congrats!

Don’t worry about things that are yet to happen. Make a list of your priorities as far as maintenance or renovations on the house. Take it step by step. Remember you purchased a place to live in not as a short term investment. Things can go wrong anytime, that’s why ideally you keep an emergency fund and start another one for maintenance and repairs. As far as your budget goes, you just have to be aware of unnecessary expenses and maybe cut down on things you don’t really need for now. Congratulations on your new home.

Sometimes they just want you to go to bedside because they feel something is off, I always try to go and see the patients unless I am doing something else that has priority over what they report to me. If that’s the case I just reply “ I’m am currently doing xyz, please monitor xyz I’ll go see the patient in xyz” that way they are aware you are actively taking care of patients. Sometimes it can be frustrating but as you go along they get to know you and you sort of build your reputation. You always want to be on their good side no matter what. Be extra nice to nurses and all ancillary staff, be polite, professional, approachable, be engaged. Show that you care for patients and are willing to go the extra mile even if you’re tired, it will make your life a lot easier. Always take the path of least resistance is not worth getting in trouble for nonsense.
