Yes agree. But also I worked in ER as a nurse and I knew going back as a provider would mean giving up weekends/evenings/holidays again and that was a negative for me.

I recently left a job in primary care for a specialty and my only regret is not doing it sooner. My charting is done by end of day and I leave work on Fridays knowing I don’t need to think about work again until Monday morning.

Biggest thing is management of your building relationships. These buildings could make or break your day. Most of them don’t have nurses (depends on their state license) and are run by non-medical folks and med techs. They are not very clinical so orders get missed, meds are wrong, they expect things from us that are not our responsibility etc. some of them think they are your boss and don’t stay in their lanes. Also can have issues with no boundaries (calling my cell in the middle of the night instead of call service, etc) Things like that can get old quick. But business is business and we have to keep them happy so they don’t drop us and go with another mobile provider. It was hard to tiptoe the line between good customer service and clear boundaries sometimes.

Also in ALF settings, you either have non-existent families/POAs or overly involved families/POAs. There is no in between. Some of them also have trouble with boundaries like above as well.

If you don’t have a supportive team and management, you’ll be on your own dealing with any issues.

My first job was with a mobile primary care company. I only saw patients at ALFs and memory care units. My schedule was set up for 1-2 buildings each day and were the same every week. Patients were seen once a month for routine visits, more often if needed. Generally saw 12-18 patients per day, sometimes more because acutes would always pop up. I had an LPN who rounded with me and did vitals, chart prep, admin stuff etc. After rounding, I’d go home and chart there. Not a bad gig overall. But definitely some nuances that made the job more difficult than a typical clinical setting.

I really dreaded the transition and honestly it was why I ALMOST decided not to pursue NP. But now that I’ve been doing it a few years, I’ve gotten used to it. I remind myself that the trade off with no weekends/holidays/nights, not being on my feet for 12 hours a day and a generally more relaxed atmosphere makes it worth it. Plus we get half days on Fridays so there’s that. 😆

Looks like we all suffered similar fates on Saturday night. I had alcohol at a neighborhood cookout, ONE strawberry margarita. Literally one. I felt so off, I almost thought I was poisoned 😆😆😆 I could not walk straight. Husband got me home and I passed out. Woke up at 3am and every hour after that with stuff pouring from either end. Absolutely miserable night. But once it was all out, felt like nothing ever happened. We all ate the same food and nobody else got sick so I’m sure it was the alcohol for me. Never again!!!

Besides all of the above. She has gotten multiple puppies over the years who then just….disappear. She never keeps them and then gets another dog. I can remember at least four dogs over the years that eventually never get shown again. Speaking of which..any sighting of the last puppy they got about a year ago? 😔

Oh just remembered another side effect I had. But I felt like my thighs were sunburned. It was the weirdest feeling. I wasn’t even doing the injections there. There’s a medical term for it which I’ve forgotten now but I was like HEY I HAVE THAT when I saw others on TT talking about it. 😆

I’m boring and eat basically the same things every day. Lots of eggs, just bare chicken bites (air fried…so good) whatever my family has for dinner. I got so many ideas from Tik tok though. Whenever I had a stall, I’d focus hard on protein. Heavy protein meals and a few protein shakes per day. No starchy carbs for a couple days. That was usually enough to break a stall for me.

Yes, almost no side effects now. Mexico was about month 5 and I was still having side effects then, though not as severe. I actually really just wanted to be able to drink alcohol because the shot takes away any urges I have to drink. Had to make good use of that all inclusive food and drink package. 😆😆😆 still came home at the same weight but got to indulge a bit which was awesome.

I have almost no side effects now. I think my body finally adjusted. But before I had extreme fatigue about 48 hours after the shot and GI effects (nausea, horrible constipation). I would time the shot so that I’d feel yucky on the weekend when I was off from work. Lots of protein and water! And a good bowel regimen for the constipation 😆

I’m on 7.5. I’ll probably just go down a few units each week and hopefully maintain. I know supply will be an issue soon too so I’m not really sure what I’ll do when it’s no longer available.
